Gowers-Zeichen vor Chorea – ein VPS13A-Fall mit schwerer Muskelschwäche

Abstract-Text (inkl. Referenzen) Einleitung: Die VPS13A-Krankheit (Chorea-Akanthozytose) ist eine seltene autosomal-rezessive neurodegenerative Erkrankung. Typisch sind ein hyperkinetisches Syndrom und gelegentlich Epilepsie. Meist persistiert eine HyperCKämie, auch wenn nicht immer neuromuskuläre Symptome bestehen. Methode: Darstellung der diagnostischen Odyssee eines Patienten mit VPS13A-Krankheit. Ergebnisse: Der jetzt 45-Jährige entwickelte mit 29 Jahren fortschreitende atrophische Paresen der Beine proximal und Arme distal mit positivem Gowers-Zeichen und Trendelenburg-Hinken. Mit 30 Jahren hatte er den ersten von vier epileptischen Anfällen. Es bestand eine HyperCKämie (900-5083 U/l). Neurographisch fand sich eine sensomotorische Polyneuropathie, myographisch neurogene, aber auch myogene Veränderungen. Die Muskelbiopsie zeigte viele atrophische COX-negative Fasern, weshalb eine Muskeldystrophie und eine Mitochondriopathie erwogen wurden. Mit 37 Jahren zeigte der Patient orofazial betonte Chorea und ein dysexekutives Syndrom. Die Exomsequenzierung ergab eine neue homozygote Frameshift-Variante in VPS13A (c.8190_8191dup, p.Glu2731Valfs*3). Bestätigend war im Western Blot das VPS13A-Produkt, Chorein, in Erythrozyten nicht nachweisbar und ein Blutausstrich zeigte 40% Akanthozyten. Diskussion: Dieser Fall unterstreicht das breite phänotypische Spektrum der seltenen VPS13A-Krankheit. Zur Diagnosestellung bei atypischer Präsentation sind gründliche (Re-)Phänotypisierung und frühe Genetik entscheidend.

Dermal nerve pathology due to HNPP

Abstract-Text (inkl. Referenzen) Introduction: We received a skin sample from the second finger of a 41 year old male patient with a positive family history of HNPP, amputated due to an injury. Methods: Besides histological examination of formalin-fixed, paraffin embedded tissue, we performed electron microscopy on glutaraldehyde-fixed, epon-embeded material to look for ultrastructural abnormalities. Results: The skin biopsy showed normal epidermal und dermal architecture with multiple dermal nerve fascicles and sensory organs (Pacini bodies). Electron microscopy revealed signs of chronic de- and remyelination, such as thinly myelinated axons, onion-bulb formations and node of Ranvier abnormalities as well as a moderate loss of unmyelinated fibers and a probably secondary axonal component. No tomaculous fibers were found. Conclusions: Dermal nerve fascicles of the patient showed a chronic demyelinating neuropathy, which, although lacking tomaculous fibers, is in line with the diagnosis of HNPP. Our findings confirm and extend previous reports on patterns skin nerve fiber involvement in HNPP.

Fallbericht einer 5q assoziierten SMA mit fatalem Outcome im Jahr 2022

Abstract-Text (inkl. Referenzen) Einleitung Mit Aufnahme in das Neugeborenenscreening kann durch eine prä- bzw. frühsymptomatische Behandlung der Krankheitsverlauf bei 5q assoziierter Spinaler Muskelatrophie entscheidend positiv beeinflusst werden. Bislang beruht das Screening auf biallelischer Deletionssuche im SMN1-Gen und berücksichtig damit 96,4% der 5q13 assoziierten SMA. Methodik Wir berichten über einen zur U3 erkannt frühsymptomatischen Säugling mit progredienter Muskelschwäche. Sowohl NG-Screening wie auch gezielte Deletionssuche im SMN1-Gen blieben unauffällig. Differentialdiagnosen wurden daher abgeklärt; unauffällige Bildgebung und Neurometabolik bis auf niedrige alpha-1,4 Glukosidase; genetisch bestätige sich M.Pompe nicht. In der Muskelbiopsie unreifes Muster mit fetalen Muskelzellen. Ergebnisse Erst im Alter von 4 Lebensmonaten konnte mittels Whole Exome Diagnostik eine 5q assozierte SMA am SMN1 Gen festgestellt werden (bei 2 SMN2 Kopien):Beide pathogenen Varianten [c.549del p.(Lys184Serfs*29)heterozygot] und [c.815A&gt;G p.(Tyr272Cys)heterozygot] konnten in der vorherigen Deletionssuche nicht erfasst werden. Das mittlerweile schwer symptomatische Kind verstarb wenige Tage nach Therapiebeginn mit Risdiplam im Rahmen einer pulmonalen Verschlechterung mit Ateminsuffizienz. Zusammenfassung Trotz unauffälligem Screening und Deletionssuche im SMN1-Gen kann selten eine behandlungspflichtige 5q assoziierte SMA vorliegen und wirft damit die Frage wie auch Herausforderung einer zukünftig adaptierten Diagnostik auf.

FUS protein expression in the myopathology of 5q-associated spinal muscular atrophy type 3

Abstract-Text (inkl. Referenzen) Aims: Spinal muscular atrophy (SMA) is a neuromuscular disease characterized by significant reduction of SMN protein. This causes degeneration of lower motor neurons in the spinal cord and brainstem leading to weakness and muscle atrophy. Clinical severity is categorized in different subtypes (type 0-3), which is turn influenced by residual SMN level. While the genetic basis of SMA is well described, the tissue-specific molecular pathways underlying SMA are still not fully understood and identification of marker proteins in human vulnerable tissue such as skeletal muscle is still lacking. Methods: To elucidate molecular markers in SMA-diseased muscle, we performed unbiased proteomics and transcriptomics on muscle biopsies derived from 3 type 3 patients in addition to immunofluorescence including 5 additional cases. Results: Combined proteomic and transcriptomic studies unraveled FUS (a DNA/RNA-binding protein and aggregation marker) as a potential molecular marker increased in SMA muscle. Results of our immunofluorescence studies showed increased FUS protein abundance accompanied by perimyonuclear disposition of the protein in a proportion of myofibres compared to the immunoreactivity obtained in control samples. Conclusion: Our data classified FUS as a protein involved in SMA-based myopathology and support the concept of skeletal muscle as a primary tissue target of SMA. Further studies are crucial to define the role of mis-localized FUS in SMA muscle.

First clinical and myopathological description of a congenital myopathy based on a homozygous variant in TNNI2

Abstract-Text (inkl. Referenzen) Background:Troponin I, fast skeletal muscle (TNNI2 encoded by the TNNI2 gene) is the inhibitory subunit of troponin, the thin filament regulatory complex which confers calcium-sensitivity to muscle actomyosin ATPase activity. Thus, TNNI2 is involved in function and integrity of sarcomeres. Multiple dominant pathogenic variants in TNNI2 (assumed to be a gain of function) are associated with congenital distal arthrogryposis. Methods:Exome sequencing was performed to identify the cause of a congenital myopathy (without arthrogryposis and cardiac symptoms) in a 15-years old girl. Muscle biopsy studies included histology, electron microscopy, proteomic profiling and immunofluorescence. Results:Molecular genetics revealed a homozygous TNNI2 variant (c.9-1G&gt;C). Microscopic studies of the muscle biopsy underlined the myopathic changes including sarcomeric disintegration. Proteomic signature of muscle revealed decreased TNNI2 level accompanied by dysregulation of proteins beyond cytoskeleton. Immunofluorescence studies of paradigmatic proteins confirmed proteomic findings. Conclusions:Our combined data add TNNI2 to the list of recessive genes associated with congenital myopathy and to the list of neuromuscular genes following both, dominant and recessive modes of inheritance. Biochemical findings indicate a loss of function mechanism, provided comprehensive insights into the biochemical nature of the disease, and introduced the first proteome for TNNI2opathies described thus far

Young man with chronic myositis – a long history of illness

Abstract-Text (inkl. Referenzen) The diagnosis of myositis is based on clinical, serological and morphological characterization and certain subtypes are defined. Inflammatory cells in muscle biopsies are a hallmark of most myositis subtypes. We received a muscle biopsy from a young man with a massive inflammatory cell component, which was initially diagnosed as myositis. Detailed analysis of muscle pathology and reevaluation of the clinical findings revealed a chronic inflammatory disease with involvement of multiple organs. The final diagnosis of a rare hereditary immune deficiency syndrome (Griscelli syndrome type 2/ GS2) was based on experts from different fields and took several years before treatment could be started. This case demonstrate a long journey of finding a diagnosis in a patient with a rare disease.
