Introduction of the postoperative Sinus Computed Tomography-Score for the assessment of the ethmoid region in recurrence of chronic rhinosinusitis with polyposis (CRSwP) after previous sinus surgery

Vorstellung des Postoperative Sinus Computertomography (POSC)-Score zur Beurteilung der Siebbeinregion bei Rezidiv einer chronisch polypösen Rhinosinusitis (CRSwP) nach vorangegangenen NNH-Operationen

Introduction: In the case of recurrence of CRSwP, there is the option of revision sinus surgery (RSS) vs. biologica therapy. This raises the question of the extent to which ethmoid bone cells (EBC) are still present despite previous surgery. These may be a contributory factor in exacerbating local Th2-inflammation. In the present study, the extent to which EBC were left in place and not removed in previous operations was investigated. For this purpose, a score was developed to assess the CT-NNH findings after previous surgery. Methods: Preoperative CT-NNH images before RSS surgery were evaluated according to the POSC score. For the ethmoid region: 0=completely present EBC, 1=partially present EBC, 2=completely surgically removed EBC. For the recessus frontalis (RF): 0=RF patency not open,1=RF patency open. The scores of the subgroups were added to a total score. Results: 73 patients who underwent RSS between 09/22 and 10/23 were evaluated. In 72.6% of the cases a history of two previous operations, in 12.3% three and in 15.1% more than four previous operations was found. The POSC score (0/1/2) showed the following frequencies: anterior EBC right 53.4%, 39.7%, 6.8%; posterior EBC right 42.5%, 47.9%, 9.6%; anterior EBC left 49.3%, 39.7%, 11.0%; posterior EBC left 42.5%, 43.8%, 13.7%; RF right 63.0%, 37.0%; RF left 65.8%, 34.2%. In 52.2% of patients, the total score was ≤ 3 and in 9.6% ≥ 7. Discussion: In summary, many patients with recurrence of CRSwP after previous surgery had complete or partial EBC. The question arises whether RSS should be preferred in these patients to remove remaining EBC before biologica therapy. The POSC score could be a useful tool for decision making in this situation.Einleitung: Beim Rezidiv der CRSwP gibt es die Option der Re-NNH-OP vs. Biologika-Therapie. Hier stellt sich die Frage, inwieweit trotz vorangegangener NNH-OP Siebbeinzellen (SBZ) weiterhin vorhanden sind. Diese können mitursächlich sein, die lokale Th2-Inflammation zu verschlimmern. In der vorliegenden Studie wurde untersucht, inwieweit SBZ in vorangegangenen Operationen belassen und nicht entfernt wurden. Hierzu wurde ein Score zur Beurteilung der CT-NNH Befunde bei Z.n. Vor-OP entwickelt. Methode: Es wurden präoperative CT-NNH Bilder vor Re-NNH-OP nach dem POSC-Score ausgewertet. Für die Siebbeinregion: 0=vollständig vorhandene SBZ, 1=teils vorhandene SBZ, 2=vollständig operativ entfernte SBZ. Für den Recessus frontalis (RF): 0=RF nicht durchgängig,1=RF durchgängig. Die Scores der Subgruppen wurden zu einem Gesamtscore addiert. Ergebnisse: Es wurden 73 Patienten ausgewertet, die zwischen 09/22 und 10/23 eine Re-NNH-OP erhielten. In 72,6% der Fälle bestand ein Z.n. zweimaliger, in 12,3% dreimaliger und in 15,1% mehr als viermaliger Vor-OP. Im POSC-Score (0/1/2) zeigten sich folgende Häufigkeiten: vordere SBZ rechts 53,4%, 39,7%, 6,8 %; hintere SBZ rechts 42,5%, 47,9%, 9,6 %; vordere SBZ links 49,3%, 39,7%, 11,0%; hintere SBZ links 42,5%, 43,8%,13,7%; RF rechts 63,0%, 37,0%; RF links 65,8%, 34,2%. In 52,2% der Patienten zeigte sich ein Gesamtscore ≤ 3 und in 9,6% ≥ 7. Diskussion: Zusammenfassend zeigt sich, dass bei vielen Patienten mit Rezidiv der CRSwP nach vorangegangener OP die SBZ vollständig oder teils vorhanden waren. Es stellt sich die Frage, ob bei diesen Patienten die Re-NNH-OP zur Ausräumung der restlichen SBZ vor einer Biologika-Therapie bevorzugt werden sollte. Der POSC-Score könnte ein nützliches Tool zur Entscheidungsfindung darstellen.Die Autorinnen/Autoren geben an, dass kein Interessenskonflikt besteht.

CRSWNP and dupilumab – real-world data after more than 2 years of clinical use

CRSWNP und Dupilumab – Real-World Daten nach über 2 Jahren klinischer Anwendung

Introduction. Dupilumab has been approved as add-on therapy for the treatment of chronic rhinosinusitis with nasal polyposis (CRSwNP) since 2019. The aim of this study is to investigate compliance, associated comorbidities, symptom development and side effects after several years of clinical use. Material and methods A total of 72 patients were included in the retrospective study between 07/2020 and 08/2023. Endpoints were changes in the SNOT-22 questionnaire, the nasal polyp score (NPS), the olfactory test (GT) using Sniffin Sticks, and the blood eosinophil concentration. The observation period extended from T0 (baseline) to 3, 6, 12 and 24 months after the start of therapy (T1-4). Comorbidities were also recorded. Results 45.9% of the patients had comorbid asthma, 30.6% had NSAID hypersensitivity , 7 patients were lost to follow-up, 2 patients continued therapy with a practicing colleague, 6 patients switched therapy and 4 patients discontinued therapy. Initially, the SNOT-22 showed a mean score of 58.1/110 (n=71), the NPS was 5.1/8 (n=72), the GT was 3.9/12 (n=17), the blood eosinophil concentration was 0.44/nl. The three, six, 12 and 24 month course showed a highly significant improvement in SNOT-22, NPS and GT. Blood eosinophil levels did not change significantly. Conclusion In patients with CRSwNP, dupilumab leads to a reduction in existing polyps and a significant reduction in sinonasal symptoms in the long term, accompanied by an improvement in olfactory performance. Anamnestic evidence shows good applicability with a low side effect profile.Einleitung Dupilumab ist seit 2019 als Add-on-Therapie zur Behandlung der chronischen Rhinosinusitis mit nasaler Polyposis (CRSwNP) zugelassen. Ziel dieser Studie ist es, die Compliance, assoziierte Komorbiditäten, Symptomentwicklung und Nebenwirkungen nach mehrjähriger klinischer Anwendung zu untersuchen. Material und Methoden Insgesamt konnten zwischen 07/2020 und 08/2023 72 Patienten in die retrospektive Studie eingeschlossen werden. Endpunkte waren Veränderungen im SNOT-22-Fragebogen, im Nasal-Polyp-Score (NPS), in der Geruchstestung (GT) mittels Sniffin Sticks sowie die Eosinophilenkonzentration im Blut. Der Beobachtungszeitraum erstreckte sich von T0 (initial) über 3, 6, 12 und 24 Monate nach Therapiestart (T1-4). Zudem wurden die Komorbiditäten erfasst. Ergebnisse 45,9% der Patienten leiden unter einem komorbidem Asthma, 30,6% unter einer ASS-Intoleranz, 7 Patienten sind lost to follow-up, 2 führten die Therapie beim niedergelassenen Kollegen fort, in 6 Fällen erfolgte ein Therapie-Switch und in 4 Fällen wurde die Therapie eingestellt. Initial ergab der SNOT-22 eine mittlere Punktzahl von 58,1/110 (n=71), der NPS lag bei 5,1/8 (n=72), die GT ergab 3,9/12 (n=17) richtig angegebene Geruchsstoffe, die Eosinophilenkonzentration im Blut lag bei 0,44/nl. Der drei-, sechs-, 12- und 24-monatigen Verlauf ergab eine hochsignifikante Besserung im Bereich des SNOT-22, des NPS und der GT. Die Eosinophilenkonzentration im Blut zeigte keine signifikante Änderung. Schlussfolgerung Dupilumab führt bei Patienten mit CRSwNP im Langzeitverlauf zu einer Reduktion vorhandener Polypen und einem deutlichen Rückgang der sinunasalen Beschwerden, begleitet von einer verbesserten Geruchsleistung. NSAID-ERD und Krankheitslast bei Patienten mit chronischer Rhinosinusitis mit Nasenpolypen im globalen Register AROMA

Introduction: Patients with chronic rhinosinusitis with nasal polyps (CRSwNP) and coexisting non-steroidal anti-inflammatory−exacerbated respiratory disease (NSAID-ERD) have more severe CRSwNP and lower quality of life. This study characterizes CRSwNP patients with NSAID-ERD initiating dupilumab in real-world practice. Methods: AROMA (NCT04959448) is a prospective global registry, recruiting adults with CRSwNP initiating dupilumab. Quality of life assessments included the 22‑item Sino-Nasal Outcome Test (SNOT-22; range 0–110) and total symptom score (TSS; 0–9). Results: As of February 2023, the study had recruited 303 patients (mean [SD] SNOT-22 and TSS of 45.4 [22.2] and 5.4 [2.3], respectively). Among patients self-reporting history of NSAID-ERD (n=78), scores were 46.5 (21.5) and 5.7 (2.2), respectively. Mean (SD) age at NSAID-ERD diagnosis was 39.3 (11.8) years. Of the patients with previous surgery (n=187), 51 (27.3%) had NSAID-ERD. Among all patients with NSAID-ERD, 11 (14.1%) had undergone confirmatory aspirin challenge, 11 (14.1%) aspirin desensitization, and 15 (19.2%) reported adverse reaction to alcohol consumption. Conclusion: A quarter of adults with CRSwNP initiating dupilumab in AROMA report having NSAID-ERD, many with high disease burden. However, most have not undergone aspirin challenge or desensitization to confirm their diagnosis.Einleitung: Bei Patienten mit chronischer Rhinosinusitis mit Nasenpolypen (CRSwNP) und gleichzeitiger Aspirin-Intoleranz (non-steroidal anti-inflammatory exacerbated respiratory disease, NSAID-ERD) ist die CRSwNP schwerer und die Lebensqualität geringer. Diese Beobachtungsstudie beschreibt CRSwNP-Patienten mit NSAID-ERD unter Alltagsbedingungen, die eine Dupilumab-Behandlung beginnen. Methode: AROMA (NCT04959448) ist ein prospektives globales Register, das Erwachsene mit CRSwNP zu Beginn ihrer Dupilumab-Behandlung aufnimmt. Zu Studienbeginn erfolgte eine Beurteilung der Lebensqualität mit dem SNOT-22 (Sino-Nasal Outcome Test, Spanne 0–110) und einem TSS (total symptom score, Spanne 0–9). Ergebnisse: Bis Februar 2023 wurden 303 Patienten in das Register aufgenommen (mittlerer [SD] SNOT-22 und TSS von 45,4 [22,2] bzw. 5,4 [2,3]). Bei Patienten mit selbst berichteter NSAID-ERD in der Anamnese (n = 78) betrugen die Werte 46,5 (21,5) bzw. 5,7 (2.2). Das mittlere (SD) Alter bei NSAID-ERD-Diagnose lag bei 39,3 (11,8) Jahren. Von den voroperierten Patienten (n = 187) hatten 51 (27,3 %) NSAID-ERD. 11 (14,1 %) der Patienten mit NSAID-ERD hatten sich zur Bestätigung einer Aspirin-Exposition und 11 (14,1 %) einer Aspirin-Desensibilisierung unterzogen. 15 (19,2 %) berichteten über Nebenwirkungen bei Akoholkonsum. Schlussfolgerung: Ein Viertel der Erwachsenen mit CRSwNP, die eine Dupilumab-Behandlung beginnen, gaben im Register AROMA an, NSAID-ERD zu haben, viele davon mit hoher Krankheitslast. Impact of type 2 inflammation on the sino-nasal and general quality of life in diffuse primary chronic rhinosinusitis

Einfluss der Typ-2 Entzündung auf die sinugene und allgemeine Lebensqualität bei primärer diffusen chronischen Rhinosinusitis

Introduction Diffuse primary chronic rhinosinisitis (CRS) is a widely spread disease with a prevalence of 5.5-28% among the European population, often resulting in a lower quality of life (QoL). Since the EPOS-guidelines 2020 CRS has been subclassified into a type 2 and non-type 2 endotype. The purpose of the current study was to investigate whether the endotype correlates with a certain impact of the patients QoL. Methods In this prospective study we included all CRS patients who underwent functional endoscopic sinus surgery (FESS) between 13.09.2018 and 12.03.2020 at the Department of Otorhinolaryngology at University Hospital Bochum. For the quantification of symptoms severity and the QoL impact we used the SNOT-20 questionnaire as well as a visual analog scale (VAS). Results We included 192 patients between the age of 18-85 with an average age of 41.8 (74 female, 118 male). 107 (55.7%) were diagnosed with CRS with nasal polyps (CRSwNP) and 84 (78.5%) of whom showed a type 2 inflammation. The SNOT-20 median value was 37.1 (standard deviation ± 16,7) and 92 patients (47.9%) documented severe symptoms (SNOT&gt;36.4). SNOT and VAS-scores did not differ between patients suffering from chronic rhinosinusitis without nasal polyps (CRSwNP), patients diagnosed with type 2 inflammation and those without. Type 2 CRSwNP patients did report a significantly higher impact on their QoL concerning nasal symptoms (p=0.002), especially the severity of rhinorrhea (p&lt;0.001) and hyposmia (p=0.001). CRSsNP patients reported more severe rhinorrhea symptoms than those with non-type 2 CRSwNP (p&lt;0.001). Conclusion The clinical endotype of CRSwNP with type 2 inflammation suffer significantly more from nasal symptoms, especially from rhinorrhea and hyposmia. Einleitung Die primäre diffuse chronische Rhinosinusitis (CRS) ist eine häufige Krankheit mit einer Prävalenz von 5,5-28%. Seit der EPOS-Leitlinie 2020, kann CRS klinisch in 2 Endotypen unterteilt werden: Typ-2 und Nicht-Typ-2. Das Ziel dieser Studie war es, zu untersuchen ob die Endotypen einen Einfluss auf die Lebensqualität haben. Methoden Es handelt sich um eine prospektive Studie, bei der CRS-Patienten eingeschlossen wurden, die von 09/2018 bis 03/2020 ins St. Elisabeth-Hospital Bochum zur FESS eingewiesen wurden. Die Quantifizierung der gesundheitsbezogenen Lebensqualität der Patienten wurde mithilfe des SNOT-20 und einer visuellen Analogskala (VAS) durchgeführt. Ergebnisse Eingeschlossen wurden 192 Patienten mit CRS von 18-85 Jahren, 74 (38,5%) waren weiblich, bei 107 (55,7%) von ihnen wurde eine CRSwNP diagnostiziert, von welchen 84 (78,5%) Endotyp 2 aufwiesen. Der mediane SNOT-Score betrug 37,1 mit einer Standardabweichung von 16,7. 92 (47,9%) der Patienten wiesen eine schwere Symptomatik auf (SNOT&gt;= 36,4). Es gab keinen Unterschied im Bezug auf die Lebensqualität zwischen Patienten mit Typ 2 CRSwNP und Nicht-Typ 2-CRSwNP. Allerdings konnte ein signifikant höherer Leidensdruck unter den primär nasalen Symptomen (p = 0,002) bei Patienten mit CRSwNP mit einer Typ 2 Entzündung festgestellt werden. Patienten mit CRSsNP gaben signifikant mehr Beschwerden an über ständiges Nasenlaufen als Patienten mit Nicht-Typ-2 CRSwNP (p &lt; 0,001). Schlussfolgerung Der klinische Endotyp von CRSwNP mit Typ-2 Inflammation, wie durch EPOS/EUFOREA in 2023 definiiert, ist signifikant mehr durch primär nasale Symptome belastet, insbesondere ständiges Nasenlaufen und Riechminderung. Er weist keine signifikant schlechtere sinugen Lebensqualität auf als bei CRSwNP ohne Typ-2-Entzündung.Die Autorinnen/Autoren geben an, dass kein Interessenkonflikt besteht.

Blindness caused by orbital apex syndrome due to uncertain inflammatory condition following functional endoscopic sinus surgery

Erblindung durch Apex orbitae Syndrom unklarer inflammatorischer Genese nach FESS

Case: The patient first presented with progressive vision loss as well as double vision, head ache and frontal loss of sensibility in July 2023. Four weeks ahead he underwent sinus surgery, the histological and microbiological examinations brought up the presence of streptococcus constallatus. Due to complete vision loss, ophthalmolplegia and frontal loss of sensibility he was diagnosed with orbital apex syndrome of unknown cause. CT and MRI scans showed a compression of the right optic nerve by an expanding leasion around the posterior orbital funnel. More scans as well as microbiological examinations of the right sphenoid sinus were performed and displayed Pseudomonas aeruginosa and MRSA. The patient was therefore treated with intravenous cefepime and linezolid. On the 7th of September a transnasal navigated biopsy of the unclear orbital mass was performed, the histological results showed granulomatous plasma cell-rich infiltration without signs of malignancy. Tests on HSV, CMV, EBV, treponema pallidum and fungal infiltration came out negative. Neurological and rheumatological consultations recommended a treatment with prednisolone, which led to a constant improvement of the symptoms as well as a shrinking of the orbital mass in following MRI scans. Recent findings point towards a possible IgG4-related diesease. More follow-ups as well as the histological examination from the first sinus surgery will be considered to provide more information on a possible diagnosis. Fall: Die initiale Vorstellung des Patienten erfolgte im Juli 2023 mit progredienter Visusminderung, Doppelbildern, Cephalgie und frontaler Sensibilitätsstörung rechts. Vier Wochen zuvor war alio loco eine beidseitige FESS erfolgt, die Histologie sowie Mikrobiologie ergaben hier lediglich den Nachweis von Strp. constallatus. Aufgrund der Kombination von nun vollständiger Erblindung sowie Ophthalmoplegie und frontaler Sensibilitätsstörung wurde die Diagnose eines Apex orbitae Syndroms unklarer Genese gestellt. Die angeschlossene Bildgebung mittels cCT und MRT der Orbita ergaben Anhalt für die Kompression des rechten Sehnervs durch einen unklaren raumfordernden Prozess im Bereich des posterioren Orbitatrichters. In domo erfolgte nach ergänzender Bildgebung auch die mikrobiologische Untersuchung des rechten Sinus sphenoidalis, hier konnten Pseudomonas aeruginosa sowie MRSA nachgewiesen werden. Der Patient wurde daraufhin nach Antibiogramm intravenös mit Cefepim und Linezolid anbehandelt. Am 07.09.2023 erfolgte komplikationslos die transnasale navigierte Biopsie des unklaren periorbitalen Prozesses. Histologisch ließen sich chronisch-granulierende, plasmazellreiche Infiltrate ohne Anhalt für Malignität nachweisen. Testverfahren auf HSV, CMV, EBV sowie Treponema pallidum oder Pilzhyphen fielen ebenfalls negativ aus. Im Verlauf wurde der Patient neurologisch sowie rheumatologisch zur weiteren Diagnostik angebunden, unter Therapie mit absteigendem Prednisolonschema kam es zu einer stetigen Besserung der Symptomatik mit ebenfalls MRT-morphologischer Größenregredienz der Raumforderung. Aktuell besteht Verdacht auf eine IgG4-vermittelte Erkrankung. Weitere MRTs sowie die histologische Aufarbeitung des initial entnommenen Gewebes werden zur Diagnosefindung herangezogen werden.Die Autorinnen/Autoren geben an, dass kein Interessenkonflikt besteht.

Case presentation of a patient with severe nasal polyposis: Use of primary biologic therapy without prior nasal sinus surgery

Fallpräsentation eines Patienten mit einer schweren Nasalen Polyposis: Einsatz einer primären Biologikatherapie ohne vorherige NNH-Operation

Introduction: Biologic therapies represent new, highly effective treatment options for severe uncontrolled chronic rhinosinusitis with nasal polyposis (CRSwNP). In accordance with international and national recommendations and the eligibility criteria of biologics in the indication CRSwNP, biologic therapies are typically performed in patients who have previously undergone nasal sinus surgery and still have no control of the disease. In the case presentation, a patient with severe nasal polyposis will be presented with a primary biologic therapy. Method: Case presentation. Results: Presentation of a 37-year-old male patient with CRSwNP, asthma and NSAID-exacerbated respiratory disease (N-ERD). Prior treatment with topical or oral corticosteroids had not shown sufficient effect. Initial findings: nasal polyposis (NP)-score of 8, total IgE 295 kU/l, eosinophils 2.2%. Due to severe anaphylaxis to atracurium, a non-depolarizing muscle relaxant, a nasal sinus surgery was not performed. After allergological diagnostics, anesthesiological consultation and at the patient's request, the patient was primarily adjusted to dupilumab (anti-IL-4Ra antibody). In the follow-up, a polyposis score of 1 was examined. A demonstration of the clinical findings will be given. Conclusion: The present case illustrates that in patients with severe nasal polyposis, primary biologic therapy without prior nasal sinus surgery is indicated in individual cases, as in the present case after severe anaphylactic reaction to muscle relaxants.Einleitung: Biologikatherapien stellen neue hocheffektive Therapieoptionen der schweren unkontrollierten Chronischen Rhinosinusitis mit nasaler Polyposis (CRSwNP) dar. Entsprechend internationaler und nationaler Empfehlungen und der Zulassungskriterien von Biologika in der Indikation CRSwNP erfolgen Biologikatherapien typischerweise bei Patienten, die im Vorfeld eine NNH-Operation erhalten haben und sich dennoch keine Kontrolle der Erkrankung einstellt. In der Fallpräsentation erfolgt eine Vorstellung eines Patienten mit einer schweren Nasalen Polyposis mit einer primären Biologikaeinstellung. Methode: Fallpräsentation. Ergebnisse: Vorstellung eines 37-jährigen Patienten mit einer CRSwNP, Asthma und einer ASS-Intoleranz. Eine Vorbehandlung mit topischen/oralen Kortikosteroiden hatte keinen ausreichenden Effekt gezeigt. Initiale Befunde: Nasaler Polyposis (NP)-Score von 8, Ges IgE 295 kU/l, Eosinophile 2,2 %. Alio loco war bei Einleitung der Narkose nach Auftreten einer anaphylaktischen Reaktion auf Atracurium, ein nicht-depolarisierendes Muskelrelaxanx, eine OP abgebrochen worden und somit eine NNH-Operation nicht erfolgt. Nach allergologischer Diagnostik, anästhesiologischem Konsil und auf Wunsch des Patienten erfolgte eine primäre Einstellung des Patienten auf Dupilumab (Anti-IL-4Ra Antikörper). Im weiteren Verlauf zeigte sich ein NP-Score von 1. Nebenwirkungen unter der Dupilumabtherapie zeigten sich nicht. Eine Demonstration der klinischen Befunde wird gegeben. Does nasal airway volume associated with increased risk of orbital complications with acute sinusitis?

Introduction: We aim to investigate the associations between nasal airway volume and the orbital complications with acute sinusitis. Materials and methods: Patients who were diagnosed with orbital complications as a result of acute sinusitis labeled as case group, and the patients who had undergone computed tomography (CT) for indications other than sinonasal diseases were assigned as control group. We compared the similar age ranged 13 patients from case group to 13 participants from control group.Tomographic volumetric measurements in five anatomical regions (the nasal cavity, maxillary sinuses, frontal sinuses, ethmoid sinuses and nasopharynx) were compared and correlated. An independent sample t -test was used to assess the association between the 3D measurements of the nasal airways. Results: Comparison between normal subjects and patients with orbital complications revealed that the case group had decreased total nasal cavity volume (P = .003), decreased ethmoid sinus volume (P = .008), decreased nasopharynx volume (P = .046) than normal subjects. The volume of the frontal sinus and maxillary sinuses were not associated with orbital complications with acute sinusitis (P &gt; 0.05). In the case group, there was no significant difference between right and left maxillary sinuses volume of complicated and healthy side. Conclusion: Decreased nasal airway, ethmoid sinuses and nasopharynx volume is associated with orbital complications of acute sinusitis. It is important to be more careful in terms of orbital complications in acute sinusitis patients with narrow nasal air passage and with adenoid hypertrophy.The authors declare that there is no conflict of interest

Dupilumab therapy for chronic rhinosinusitis with nasal polyps (cRSwNP) in pregnancy

Dupilumab-Therapie bei chronischer Rhinosinusitis mit Nasenpolypen (cRSwNP) in der Schwangerschaft

Background: Rescue therapy with monoclonal antibodies for cRSwNP is now an integral part of rhinologic therapy. Due to the current state of research, it is recommended to suspend the therapy in the case of pregnancy. Research question: Does therapy with dupilumab have an effect on the course of pregnancy or the health of the child? Material and methods: We report on a 32-year-old female patient with cRSwNP (history of 2x pansinus operation), who had applied eight doses of dupilumab 300mg until she became aware of a pregnancy with 16 weeks of gestation. The course of pregnancy, birth and first weeks of life of the child were followed and a literature search was carried out on all known cases of pregnancy under dupilumab therapy. Results: The patient (gravida II, para I) had a premature birth in the 34+5th week of gestation with premature rupture of membranes, analogous to the first child (not under dupilumab). The child (weight: 80. percentile (pct.); length: 54. pct.; head circumference: 41. pct.) required CPAP respiratory support for four hours and concomitant partial parenteral nutrition for nine days. The further development of the child was regular (14 weeks old in November 2023). The literature search identified seven studies on dupilumab therapy (for atopic dermatitis and bronchial asthma) with reports of pregnancy (N=54). A further 22 case reports were also found. Conclusion: Our experience and the research data suggest that the use of dupilumab is also safe in pregnancy. Preterm birth occurred in both Para I (without dupilumab) and Para II (with dupilumab). Our observations may support the decision-making when considering the continuation of dupilumab therapy during pregnancy.Hintergrund: Die Rescue-Therapie mit monoklonalen Antikörpern bei cRSwNP ist inzwischen fester Bestandteil der rhinologischen Therapie. Aufgrund der schwachen Studienlage wird aktuell empfohlen, die Therapie im Falle einer Schwangerschaft (SS) auszusetzen. Fragestellung: Hat die Therapie mit Dupilumab Auswirkungen auf den SS-Verlauf oder die Gesundheit des Kindes? Methodik: Wir berichten über eine 32-jährige Patientin mit cRSwNP (Z.n. 2x-maliger Pansinus-Operation), die bis zum Bekanntwerden einer SS in der 16. SSW acht Gaben Dupilumab 300mg appliziert hatte. Der Verlauf von SS, Geburt und ersten Lebenswochen des Kindes wurden verfolgt, sowie eine Literaturrecherche zu allen bekannten Fällen von SS unter Dupilumab durchgeführt. Ergebnisse: Bei der Patientin (Gravida II, Para I) kam es zu einer Frühgeburt in der 34+5. SSW bei vorzeitigem Blasensprung, analog zum ersten Kind (nicht unter Dupilumab). Das Kind (Gewicht 80. Perzentile (Pzt.); Länge: 54. Pzt.; Kopfumfang: 41. Pzt.) benötigte eine CPAP-Atemunterstützung für vier Stunden sowie eine begleitende teilparenterale Ernährung für neun Tage. Die weitere Entwicklung des Kindes war regelhaft (im November 2023 14 Wochen alt). In der Literaturrecherche konnten sieben Studien zu Dupilumab Therapie (bei atopischer Dermatitis und Asthma bronchiale) mit Berichten von SS (N=54) identifiziert werden. Es fanden sich außerdem 22 weitere Fallberichte. Schlussfolgerung: Unsere Erfahrungen und die Daten der Literatur lassen vermuten, dass die Anwendung von Dupilumab auch in der SS sicher ist. Frühgeburtlichkeit bestand sowohl bei Para I (ohne Dupilumab) als auch bei Para II (mit Dupilumab). Impact of CRS endotype on asthma-related quality of life

Introduction Chronic Rhinosinusitis (CRS) may manifest phenotypically with or without nasal polyps (CRSw/sNP) and can be endotypically classified into Type 2 or Non-Type 2. Patients with Type 2 CRSwNP exhibit the highest risk of comorbid asthma. The objective of this study was to analyze sinonasal and asthma-related quality of life (QoL) in patients with comorbid asthma and primary diffuse CRS. Methods Between September 2018 and March 2020, all patients with asthma and CRS undergoing surgery at the ENT University Clinic in Bochum were prospectively enrolled. Sinonasal QoL was objectified using the SNOT20-GAV and the 'Primary Nasal Symptoms' (PNS) subscore. AQLQ and ACQ were employed to assess asthma-related QoL and asthma control, respectively, with FEV1 predicted value serving as an objective measurement. Results A total of 39 patients were included, with 26 presenting with Type 2 CRSwNP. AQLQ and ACQ scores, as well as the FEV1 predicted value, did not demonstrate inferior outcomes in patients with Type 2 CRSwNP. In patients with Type 2 CRSwNP, a significant association between SNOT and ACQ scores was observed (p = 0.01). PNS also exhibited a significant association with ACQ score (p = 0.047). This association was not observed in patients with Non-Type 2 CRSwNP or CRSsNP. The SNOT scale was significantly associated with the AQLQ score in all CRS endotypes (p &lt; 0.001). Discussion Asthma-related QoL and FEV1 predicted value remain unaffected by comorbid CRS endotype. In patients with Type 2 CRSwNP, the severity of primary sinonasal symptoms correlates with perceived asthma control, lending support to the concept of United Airway Disease. This correlation is absent in other endotypes, suggesting that asthma in these subgroups could be regarded as a distinct entity. Patienten mit Typ-2- CRSwNP zeigen das höchste Risiko eines komorbiden Asthmas. Das Ziel dieses Projekt war es, die sinunasale und asthmabedingte Lebensqualität (LQ) bei Patienten mit komorbidem Asthma und primärer diffuser CRS zu analysieren. Methoden Zwischen September 2018 und März 2020 wurden alle Patienten mit Asthma und CRS, die sich einer Operation in der HNO-Universitätsklinik in Bochum unterzogen, prospektiv eingeschlossen.
