The predictive value of the sentinel lymph node biopsy in malignant melanoma of the head and neck in the Kiel patient collective

Introduction: Sentinel lymph node biopsy (SLNB) has been an integral part of the diagnosis and staging of malignant melanoma (MM) since the end of the last millennium. The indication for a sentinel lymph node biopsy is a tumor thickness of 1.0 mm or more in the absence of locoregional or distant metastases. A sentinel lymph node biopsy can also be performed in the case of a thinner primary tumor (0.75-1.0 mm) if there is an increased risk of occult lymph node metastases, e.g. in the case of an increased mitosis rate, young age or ulceration of the primary tumor. The positivity of a SLN has a significant influence on the subsequent therapy and prognosis. The aim of this retrospective analysis is, among other things, to determine the predictive value of the biopsy against the background of the actual course of the patient's disease. Of particular interest is the frequency of SLNs without metastasis in patients who nevertheless develop metastases in the course of the disease. Materials and Methods: For this purpose the data of patients who were treated in the ENT Clinic of the UKSH-Kiel between 2002 and 2020 are evaluated. For the time being 117 patient histories have been analyzed retrospectively. Results: The detection rate of sentinel lymph nodes is 88.89% (111 of 117). Locoregional metastases occurred in 5 out of 81 patients with initially negative sentinel lymph nodes (6.17%). The sensitivity is 85.71%. The false-negative rate is 16.67%. The negative-predictive value is 94.7%. Conclusion: SLNB continues to be an important component in the diagnosis of malignant melanoma in the head and neck region. However, the treating team should be aware of the limitations of the diagnostic procedure sometimes resulting in insufficient treatment.Die Wächterlymphknotenbiopsie (WLKB) stellt seit Ende des letzten Jahrtausends einen integralen Teil bei der Diagnosestellung und Bestimmung des Stadiums des malignen Melanoms (MM) dar. Die Indikation zur Wächterlymphknotenbiopsie besteht bei einer Tumordicke ab 1,0 mm und Abwesenheit von lokoregionären oder Fernmetastasen. Ebenfalls kann eine WLKB bei einem dünneren Primärtumor (0,75-1,0mm) durchgeführt werden, falls ein erhöhtes Risiko für okkulte Lymphknotenmetastasen besteht, z.B. bei einer erhöhten Mitoserate, jungem Alter oder Ulzeration des Primärtumors. Die Positivität eines WLK hat signifikanten Einfluss auf die weiterfolgende Therapie und Prognose. Das Ziel dieser retrospektiven Analyse ist unter anderem der prädiktive Wert der Biopsie vor dem Hintergrund des tatsächlichen Krankheitsverlaufs der PatientInnen zur ermitteln. Von besonderem Interesse ist hierbei die Häufigkeit von WLK ohne Metastasierung bei PatientInnen, welche im Verlauf aber doch Metastasen entwickeln. Hierzu werden die Daten von PatientenInnen, welche zwischen 2002 und 2020 in der HNO-Klinik des UKSH-Kiel behandelt worden sind, ausgewertet. Vorläufig konnten 117 Patientenverläufe retrospektiv ausgewertet werden. Die Detektionsrate der Wächterlymphknoten liegt bei 88,89% (111 von 117). Lokoregionäre Metastasen sind im Verlauf bei 5 von 81 Patienten mit initial negativem WLK aufgetreten (6,17 %). Die Sensitivität liegt bei 85,71%. Die falsch-negativ Rate beträgt 16,67%. Der negativ-prädiktive Wert beträgt 94,7%. Die WLKB ist weiterhin ein wichtiger Bestandteil in der Diagnosestellung des malignen Melanoms im Kopf-Halsbereich. Den Behandelnden sollten jedoch die Limitationen des diagnostischen Verfahrens und die ggf. daraus resultierende unzureichende Therapie von PatientInnen bewusst sein.Die Autoren geben an, dass kein Interessenkonflikt besteht

T-VEC in the palliative treatment of Mucosal Melanoma

T-VEC Injektion im palliativen Setting von Schleimhautmelanomen

Introduction: Intralesional injection of oncolytic viral immunotherapies leads to apoptosis of tumour cells and induction of an immune response. Talimogene laherparepvec (T-VEC) is approved for the treatment of malignant melanoma in non-resectable and metastatic settings. Patients/Methods: We present the case of a 61-year-old patient with malignant mucosal melanoma of the nasal cavity and sinuses. After multiple previous surgeries and radiotherapy, during immunotherapy with nivolumab local tumor progression was detected. The tumor was inoperable. In accordance with the interdisciplinary tumor board, multiple endoscopically controlled local injections with T-VEC were performed an immunotherapy with nivolumab and ipilimumab was started. Results: The patient received a total of 8 intralesional injections of T-VEC. The findings showed complete regression. The regular endoscopic check-ups showed no evidence of local tumor growth over the course of 5 months. Post-interventionally, the patient suffered from flu-like symptoms with a slight fever for about 24 hours. Discussion: T-VEC is successfully used with a manageable side effect profile in the palliative setting for local tumor control. In further studies, neoadjuvant therapy for locally advanced tumours could be tested. T-VEC can be discussed as an off-label treatment option for refractory mucosal melanomas or in neoadjuvant therapyEinleitung: Die intraläsionale Injektion von onkolytischen viralen Immuntherapien induziert Apoptose in Tumorzellen sowie eine Immunreaktion. Talimogene laherparepvec (T-VEC) ist zur Behandlung in der metastasierten und nicht-resektablen Situation für das des maligne Melanom zugelassen. Patienten/Methodik: Wir stellen den Fall eines 61 jährigen Patienten mit malignem Schleimhautmelanom der Nasenhaupt- und Nebenhöhlen vor. Nach mehrfacher Voroperation und Radiatio zeigte sich unter Immuntherapie mit Nivolumab ein lokaler Tumorprogress. Eine erneute operative Resektabilität war nicht gegeben. Entsprechend des interdisziplinären Tumorkonferenzbeschlusses erfolgte eine mehrfache endoskopisch kontrollierte lokale Injektion mit T-VEC, parallel wurde eine Immuntherapie mit Nivolumab plus Ipilimumab eingeleitet. Ergebnisse: Insgesamt erhielt der Patient 8 intraläsionale Gaben von T-VEC. Im Verlauf zeigte sich der Befund vollständig regredient. Im Rahmen der regelmäßigen endoskopischen Kontrollen zeigt sich im bisherigen Follow Up von 5 Monaten kein Anhalt für ein lokales Tumorwachstum. Postinterventionell litt der Patient über etwa 24h an grippeähnliche Symptome mit teils leichtem Fieber. Zusammenfassung: T-VEC wird erfolgreich mit überschaubarem Nebenwirkungsprofil im palliativen Setting zur lokalen Tumorkontrolle eingesetzt. Bei therapierefraktären Schleimhautmelanomen oder auch in der Neoadjuvanz kann T-VEC als off-label-Therapieotion diskutiert werdenDie Autorinnen/Autoren geben an, dass kein Interessenkonflikt besteht.

Possibilities of displaying the supraorbital and nasal plexus for preoperative flap design in complex nasal reconstruction

Möglichkeiten zur Darstellung des supraorbitalen Gefäßplexus für die präoperative Lappenplanung bei größeren Nasenrekonstruktionen

Background: The paramedian forehead flap is an axial pattern flap with frequent use in plastic reconstruction of complex nasal defects. Its main blood supply is based on the supratrochlear artery as part of the supraorbital/nasal plexus. Objectives: The aim of this study was to evaluate, to what extend the supraorbital/nasal plexus can be determined by a dynamic 4D-CT Angiography (4D-CTA) and might have an influence on the preoperative forehead flap design for nasal reconstruction. Methods: From 2014 to 2020, 16 patients with complex 2- to 3-layered nasal defects underwent dynamic 4D-CTA preoperatively (Somaton Force, Siemens, Healthineers). Results: In 15 out of 16 patients, the perfusion of the supraorbital plexus could be visualized and integrated into the preoperative reconstruction plan, which failed in one patient. The supraorbital plexus on both sides of the face demonstrated a different pattern of vascular perfusion. In 73% of the cases, this information had an influence on the preoperative flap design as the artery with the highest contrast defined the side of the forehead to be chosen. Conclusions: This study underlines the value of dynamic 4D-CTA in nasal reconstruction as it enables a better anatomical understanding of the vascular structures in the forehead region. Einleitung: Zur plastischen Rekonstruktion komplexer Nasendefekte wird oft der axial gestielte paramediane Stirnlappen eingesetzt. Die Blutversorgung erfolgt primär über die A. supratrochlearis, die zum supraorbitalen Gefäßplexus gehört. Fragestellung: Das Ziel der prospektiven Anwendungsstudie ist zu beurteilen, inwieweit eine Perfusionsanalyse des supraorbitalen Gefäßplexus durch eine dynamische 4D-CT Bildgebung für die präoperative Planung des Lappendesigns möglich ist. Material und Methoden: Über einen Zeitraum von 2014 bis 2020 wurden bisher 16 Patienten mit komplexen 2- bis 3-schichtigen Nasendefekten präoperativ einer dynamischer 4D Perfusions-CT Bildgebung (Somaton Force, Siemens, Healthineers) unterzogen. Ergebnisse: In 15 der 16 Patientenfälle konnte die Perfusion des supraorbitalen Gefäßplexus dargestellt und für die OP-Planung korreliert werden. In einem Fall war die Perfusionsdarstellung diagnostisch nicht verwertbar. Die supraorbitalen Gefäßplexus zeigten im direkten Seitenvergleich beider Gesichtshälften unterschiedliche Perfusionsmuster. Dies beeinflusste in 73% der Fälle das präoperative Lappendesign, da die Gesichtsseite mit der stärker kontrastierten Arterie in die rekonstruktive Planung einbezogen wurde. Zusammenfassung: Die dynamische 4D-Perfusions-CT Bildgebung leistet einen wertvollen präoperativen Beitrag zur anatomischen Darstellung des supraorbitalen Gefäßplexus im Rahmen komplexer Nasenrekonstruktionen. Die Autorinnen/Autoren geben an, dass kein Interessenkonflikt besteht.

The overestimation of squamous cell carcinoma of the salivary glands – A nationwide German population based study

Introduction: Primary squamous cell carcinoma of the salivary glands (pSCC) has been described, but it must be distinguished from secondary squamous cell carcinoma (sSCC). In a previous study, we found an unusually high proportion of pSCC in the North Rhine-Westphalian (NRW) Cancer Registry. We investigated whether this finding can be observed throughout Germany and whether these tumors could be misclassified sSCC. Materials/Methods: Data on all salivary gland cancers (SGC) and cutaneous SCC (cSCC) diagnosed between 2009 and 2019 were requested from the German Centre for Cancer Registry Data (ZfKD). We estimated incidence rates of pSCC and additional cSCC and compared them between federal states. We compared the relative 5-year survival of pSCC with that of other morphologies. Results: We retrieved 10079 SGC and 927 cSCC cases from the ZfKD database. Of all SGC, 2292 (22.7%) were pSCC. The proportions of pSCC among SGC in the 16 German federal states ranged from 17.4% to 27.7%. Median age at diagnosis was 78 years (SD=12.2). The sex ratio of the age-standardized pSCC incidence rates was 3.4:1 (male:female). Primary sites of pSCC were parotid gland (84.1%), submandibular gland (12.3%), sublingual gland (1.1%) and overlapping/not otherwise specified regions (2.6%). The proportion of additional cSCC was 25.6% in pSCC patients, compared to 3.7% in adenocarcinoma patients (RR = 6.91 [95%CI 6.05, 7.89]). Relative 5-year survival of pSCC was 50.2% [95%CI: 46.1%, 54.2%]. Discussion: We found evidence for a significant overestimation of pSCC incidence in all German cancer registries. This is likely due to missing clinical information about e.g previous cSCC at the time of pathological assessment leading to misclassification of secondary tumors as pSCC.Kein Interessenskonflikt
