Abstract

Introduction

Short-course antibiotics in clean-contaminated procedures such as laryngectomies reduce postoperative infections. There is no evidence for an additional value of longer antibiotic durations, and no evidence for antibiotics in clean procedures. Questionnaires on procedures in general ENT have shown practical use of antibiotics to be significantly higher and inhomogeneous. To judge the current use of antibiotics, we conducted a joint survey between the EORTC and AHNS and multiple national societies including the DGHNO.

Methods

The survey was conceived among EORTC head and neck young and early career investigators with partners at the AHNS. Ethics review was obtained from the University of Greifswald Medical. Questions focused on general demographics and specific treatment cases. Surgery was classified into clean, clean-contaminated, and complicated clean-contaminated surgery such as salvage surgery. The survey is web-based and has been released in October 2024.

Results

We will present results including the DGHNO respondents at the meeting.

Discussion

There is a significant negative effect of antibiotics on the disease specific and overall survival in patients receiving immune therapy for head and neck cancer. There has also been evidence that suggest there might be a negative impact on the disease specific survival in chemoradiation. While we expect antibiotic use in clean surgery to be lower in Germany than in some other countries, there is likely more antibiotic use in clean-contaminated procedures than warranted by evidence. We present the survey as a baseline for further prospective trials on the matter.
