Objective This study aimed to analyze the educational quality and content of septoplasty videos on YouTube and to validate the &quot;Instructional Videos in Otorhinolaryngology by YO-IFOS-Grading-System&quot; (IVORY-GS. Methods A search for &quot;septoplasty,&quot; &quot;closed septoplasty,&quot; and &quot;deviated nasal septum surgery&quot; was performed on YouTube in March 2023. Videos of surgeries on patients and cadaver dissections were included. The IVORY-GS was adjusted specifically for septoplasty and applied to rate the videos. Statistical analyses were conducted to identify associations between video features and the IVORY-GS total score. Results A total of 105 septoplasty videos were included. Among the analyzed videos, 40% demonstrated Cottle&quot;s technique, while 53% followed Killian&quot;s technique. The mean IVORY-GS total score for septoplasty videos was 25.3 (out of a maximum of 48). Video education quality was rated as moderate in 7% and high in 3%. Endoscopic videos and videos published within the last 50 months received significantly higher total scores than macroscopic videos (p &lt; 0.01) and videos published earlier (p &lt; 0.01). A statistically significant correlation was found between the total IVORY-GS score and the number of likes (p = 0.02). A higher total IVORY-GS score was a significant predictor for more likes (p = 0.02). Conclusion The analysis revealed that a majority of septoplasty videos available on YouTube demonstrate a low overall educational quality. The IVORY-GS has proven useful in assessing the educational value and can be beneficial in establishing a dedicated online platform for high-quality otorhinolaryngology surgery videos.Einleitung: Diese Studie zielte darauf ab, die didaktische Qualität und den Inhalt von Septumplastik-Videos auf YouTube zu analysieren und das "Instructional Videos in Otorhinolaryngology by YO-IFOS-Grading-System" (IVORY-GS) zu validieren. Methoden: Im März 2023 wurde auf YouTube eine Suche nach "Septumplastik", "geschlossene Septumplastik" und "Nasenscheidewanddeviation" durchgeführt. Es wurden Videos von Operationen an Patienten und Leichensektionen einbezogen. Der IVORY-GS wurde speziell für die Septumplastik angepasst und zur Bewertung der Videos verwendet. Es wurden statistische Analysen durchgeführt, um Zusammenhänge zwischen Videomerkmalen und dem IVORY-GS-Gesamtscore zu ermitteln. Ergebnisse Es wurden insgesamt 105 Septoplastik-Videos berücksichtigt. Von den analysierten Videos zeigten 40 % die Technik von Cottle, während 53 % die Technik von Killian anwandten. Die durchschnittliche IVORY-GS-Gesamtbewertung für Septoplastik-Videos betrug 25,3 (von maximal 48). Die Qualität der Videos wurde in 7 % der Fälle als mäßig und in 3 % als hoch eingestuft. Endoskopische Videos und Videos, die in den letzten 50 Monaten veröffentlicht wurden, erhielten signifikant höhere Gesamtpunktzahlen als makroskopische Videos (p &lt; 0,01) und Videos, die früher veröffentlicht wurden (p &lt; 0,01). Es wurde ein statistisch signifikanter Zusammenhang zwischen dem IVORY-GS-Gesamtscore und der Anzahl der Likes festgestellt (p = 0,02). Eine höhere IVORY-GS-Gesamtpunktzahl war ein signifikanter Prädiktor für mehr Likes (p = 0,02). Orbital cellulitis in a patient 30 years after Caldwell-Luc surgery

Introduction: Orbital cellulitis is a rare inflammatory condition that primarily affects pediatric patients as a complication of acute bacterial infections of the paranasal sinuses. Although there has been a decrease in these complications due to antibiotic therapy, it is a condition that endangers the patient's vision and it is potentially life-threatening condition. Early and appropriate treatment is necessary. Case study: Our case presents a patient who developed orbital cellulitis 30 years after undergoing Caldwell-Luc surgery. The patient with subfebrile temperature was examined at our clinic with a week-long history of increasing pain, swelling and redness in the left eye. There was no nasal symptoms. During the eye examination, limited motility and protrusion of the eyeball and diplopia in extreme positions were confirmed. On the CT scan there was an abscess collection in the left maxillary sinus with extension into the left orbit and destruction of the orbit's floor skeleton. Results: The patient underwent endoscopic surgical treatment at our clinic on the same day. Within two days post-surgery, there was a significant improvement in symptoms, and after one week the patient was completely free of any complications. Discussion: The aim of this case study is to highlight the importance of obtaining a full medical history, accurate diagnosis, and appropriate therapeutic approach. Furthermore, interdisciplinary collaboration between otolaryngologists, ophthalmologists and radiologists is crucial.The authors declare that there is no conflict of interest.

Orbital decompression and frontalis suspension surgery in unilateral exophthalmos and ptosis due to oculomotor nerve palsy

Introduction: Congenital oculomotor nerve palsies occur rarely and are classified according to the localization of the lesion. Clinically, external oculomotor nerve palsy is characterized by strabismus and ptosis. Exophthalmos may occur due to the loss of posterior traction of the rectus muscles. Methods/Material: A 15-year-old female patient presented to our clinic with right external oculomotor nerve palsy that had been present since birth. Symptomatically, the patient described pronounced sicca symptoms. Several eye muscle operations had already been performed in the local ophthalmology department without sufficient improvement of the findings. The examination revealed an exophthalmos of the right eye with a Hertel index of 18 mm (left 15 mm). The native CT of the sinuses showed age-appropriate findings. A medial decompression was performed transnasally with opening of the periorbita after ablation of the lamina papyracea. In addition, a frontalis suspension with fascia lata was performed on the affected side. Results: Postoperatively, there was a significant decrease in the Hertel index and an improvement in eyelid closure. The symptoms, especially the dryness of the eye, were regressive. Conclusion: In cases of congenital external oculomotor nerve palsy, medial orbital decompression in combination with frontal suspension can be evaluated as a treatment option. This can reduce the symptoms.Einleitung: Kongenitale Okulomotoriusparesen treten selten auf und werden unterteilt nach Lokalisation der Läsion. Klinisch zeigt sich bei der äußeren Okulomotoriusparese ein Strabismus, sowie eine Ptosis. Es kann, bedingt durch den Ausfall des Zuges der Mm. recti nach dorsal, ein Exophthalmus auftreten. Methoden/Material: Eine 15 Jahre alte Patientin stellte sich mit seit Geburt besehender äußerer Okulomotoriusparese rechts in unserer Klinik vor. Symptomatisch beschrieb die Patientin eine ausgeprägte Sicca-Symptomatik. In der hiesigen Augenheilkunde waren bereits einige Augenmuskeloperationen ohne ausreichende Befundverbesserung erfolgt. In der Untersuchung zeigte sich ein Exophthalmus rechts mit einem Hertel-Index von 18mm (links 15mm). Im nativen CT-NNH zeigten sich altersgerechte Befunde. Es erfolgte eine mediale Dekompression transnasal mit Eröffnung der Periorbita nach Abtragung der Lamina papyracea. Es wurde außerdem eine Frontalissuspension mit fascia lata auf der betroffenen Seite durchgeführt. Ergebnisse: Postoperativ zeigte sich ein deutlicher Rückgang des Hertel-Index, sowie eine Verbesserung des Lidschlusses. Die Beschwerden, insbesondere die Augentrockenheit zeigten sich regredient. Schlussfolgerung: Bei einer kongenitalen äußeren Okulomotoriusparese kann eine mediale Orbitadekompression in Kombination mit Frontalissuspension als Therapieoption evaluiert werden. Erdheim-Chester disease, a rare manifestation in the orbit

Die Erdheim-Chester-Erkrankung mit einer seltnenen Manifestation in der Orbita

Introduction: Erdheim-Chester disease (ECD) is a rare disease (less than 500 documented cases) belonging to the group of histiocytoses. The disease can affect several organ systems and the symptoms are correspondingly unspecific. Ocularly exophthalmos, visual disturbances, motility disorders, papilledema, xanthelasma and corneal manifestations have been described. Methods: 4 patients presented interdisciplinary in domo, 2 women and 2 men aged 59-62 years at initial diagnosis. All presented with uni- or bilateral exophthalmos and visual deterioration. Patient 1 (m) had already been diagnosed alio loco by tibial biopsy. Patient 2 (f) was confirmed in domo by orbital biopsy. Patient 3 (f) was diagnosed in domo by subcutaneous biopsy in the area of the mons pubis. This patient showed progression of the mass in the right orbit 2 years later. A biopsy revealed sarcomatoid tissue in the form of a histiocytic sarcoma. Patient 4 (m) was diagnosed by orbital biopsy. Results: Patients 1, 2 and 4 were successfully treated conservatively to date. Patient 1 with vemurafinib, patient 2 with prednisolone. Patient 4 initially with cytarabine and after 2 years with vemurafinib in case of exacerbation. Patient 3 underwent a right orbital exenteratio due to the invasive nature of the tumor. Discussion: We show the different ocular manifestations of the disease and their response to treatment. To our knowledge, case 3 is the first case report of ECD with ocular involvement showing malignant transformation. ECD was considered a fatal disease with a median overall survival rate of less than 40% within the first 3 years. With modern therapies, outcomes have improved. Increased awareness of screening these rare patients may therefore improve the quality of life and impact of therapy.Einleitung:Die Erdheim-Chester-Erkrankung (ECE) ist eine seltene Erkrankung aus dem Formkreis der Histiozytosen. Die Erkrankung kann mehrere Organsysteme befallen, die Symptome entsprechend unspezifisch. Okulär sind Symptome wie Exophthalmus, Sehstörungen, Motilitätsstörungen und Hornhautmanifestationen beschrieben.Methoden: Vier Patienten (2f, 2m) mit ECE stellten sich in unserem interdiziplinärem Orbitazentrum vor. Alle boten ein oder beidseitig einen Exophthalmus und eine Sehverschlechterung. Patient 1 war mittels Tibiabiopsie diagnostiziert worden. Patient 2 wurde mittels Orbita Biopsie gesichert. Patient 3 wurde mittels subcutaner Biopsie im Bereich der Mons pubis diagnostiziert. Bei dieser Patientin zeigte sich nach 2 Jahren eine Progredienz der Raumforderung der Orbita rechts. Bei einer Biopsie wurde sarkomatoides Gewebe im Sinne eines histiozytären Sarkoms gesichert. Patient 4 wurde mittels Orbitabiopsie diagnostiziert.Ergebnisse:Patient 1,2 und 4 konnten erfolgreich konservativ behandelt werden. Patient 1 mittels Vemurafinib, Patient 2 mittels Prednisolon, Patient 4 zunächst mit Cytarabin und bei Exazerbation nach 2 Jahren mit Vemurafinib. Bei Patient 3 musste auf Grund des invasiven Charakters des Tumors eine Exenteratio Orbitae durchgeführt werden. Diskussion:Wir zeigen die verschiedenen okulären Erscheinungsformen der Krankheit und ihr Ansprechen auf die Therapie. Fall 3 ist nach unserem Kenntnisstand der erste Fallbericht einer Augenbeteiligung mit maligner Entartung. ECE galt als tödliche Krankheit mit einer medianen Gesamtüberlebensrate von weniger als 40 % innerhalb der ersten 3 Jahre. Mit modernen Therapien haben sich die Ergebnisse verbessert. Eine Sensibilisierung für diese seltene Erkrankung kann daher die Lebensqualität und die Therapie verbessernes besteht kein Interessenskonflikt

Closure of nasal septum perforation: no unwanted changes to the appearance of the external nose

Introduction: A septal perforation can be closed by septal perforation closure with bridge flaps. This is done through an open or a closed approach and with harvesting of ear cartilage. In this study, we investigated whether the surgery causes any change in the appearance of the external nose and auricle. Methods: Ten patients with septal perforation who underwent septal perforation closure with bridge flap according to Schultz Coulon in the ENT department of the University Hospital Essen between 2021 and 2023 were included. Photo documentation was performed pre- and 3 months postoperatively, which was evaluated using a scale. Results: To date, 10 patients with complete data sets have been included in this study. In 2 patients surgery was performed via a closed approach. In all patients, ear cartilage was harvested from the right ear. In one patient, a columella strut was also performed because of the lack of protection of the nasal tip due to the septal perforation. In addition, in one patient, a modification of the external nose (cusp ablation) had been requested by the patient. On average, the length of the columella was 27.5 mm preoperatively and 28.2 mm postoperatively. The head-helix distance in the mid-helix region was 22.3 mm preoperatively and 21.7 mm postoperatively. Conclusion: There were no significant unwanted changes in the appearance of the external nose or auricle.Einleitung: Eine Septumperforation kann durch Septumperforationsverschluss mit Brückenlappen verschlossen werden. Dies erfolgt über einen offenen oder einen geschlossenen Zugang und unter Entnahme von Ohrknorpel. In dieser Studie wurde untersucht, ob die Operation eine Veränderung der äußeren Nase und der Ohrmuschel bewirkt. Methoden: Es wurden 10 Patienten mit Septumperforation, die in der HNO des Universitätsklinikums Essen zwischen 2021 und 2023 einen Septumperforationsverschluss mit Brückenlappen nach Schultz Coulon erhielten, eingeschlossen. Es erfolgte prä- und 3 Monate postoperativ eine Fotodokumentation, welche mithilfe eines Maßstabs ausgewertet wurde. Ergebnisse: Bislang wurden 10 Patienten mit vollständigem Datensatz in diese Studie eingeschlossen. Bei 2 Patienten erfolgte die Operation über einen geschlossenen Zugang. Zudem war bei einem Patienten eine Änderung der äußeren Nase (Höckerabtragung) seitens des Patienten gewünscht worden. Im Durchschnitt betrug die Länge der Columella 27,5 mm präoperativ und 28,2 mm postoperativ. Der Kopf-Helix-Abstand im mittleren Bereich der Helix lag präoperativ bei 22,3 mm und postoperativ bei 21,7 mm. Schlussfolgerung: Es ergaben sich keine signifikanten ungewollten Veränderungen der äußeren Nase oder der Ohrmuschel.Die Autorinnen/Autoren geben an, dass kein Interessenkonflikt besteht.

Introduction It is known that the oblique eye axis plays a severe role in the perception of beauty and there are gender- and age-specific differences in what is considered attractive. With the elevated influence of social media on young patients, current beauty standards are portrayed and compared on the ordinary. As a result there are less inhibitions on undergoing medical procedures in order to achieve ones wishes regarding the facial appearance. The aim of this analysis is the description of contemporary trends in eye shapes. Material and Methods A trend analysis of google research terms was performed. Peaking search items were extendedly researched on the social media platform TikTok. In addition, a literature research was performed. Results Fox eyes: Lift of the lateral eyebrow - achieved through thread insertion Hunter eye: Considered a dominant and masculine periocular aesthetic positive canthal tilt, eyelid exposure little to none, deep set eyes, straight eyebrows ≠ prey eye Doe eye: Large, round eye shape, feminine, gentle, innocent Discussion Periocular aesthetic relates to facial balance and harmony and can influence the perception of the entire face. Different attributes are associated with different eye shapes. Surprisingly, binary gender roles are once again being subdivided and given attributes such as strong for males and soft in female persons. Young people in particular often consume these trends and representations unfiltered. International comparison of outpatient vs. inpatient treatment of patients with nasal packing

Ambulantes vs. stationäres Management von Patienten mit Nasentamponaden im internationalen Vergleich

Introduction: Nasal packing plays an important role in the treatment of epistaxis, whether it occurs iatrogenically after surgery or spontaneously or due to other primary diseases. A feared, albeit rare complication is the dislocation of the nasal packing, for which lethal courses have been described in the literature. Because of this, there is a great reluctance in Germany about the outpatient management of patients with nasal packing. Material/Method: A literature search in the guidelines of medical societies in nations with comparable health standards was performed with regard to recommendations for the management of patients with nasal packing. Results: Looking at other European and non-European countries reveals that the German practice is not widely shared. Rather, we find a much more differentiated situation. Nasal packing is often not considered as an obstacle to discharge. Moreover, studies show that there is evidence against the mandatory inpatient treatment of patients with nasal packing, as long as important risk factors have been excluded in the patients. Discussion: Internationally, the common practice in Germany of not discharging patients with nasal packing has little accordance. Therefore, studies comparing patient safety and treatment success between outpatient and inpatient patients with nasal packing would be desirable.Einleitung: In der Behandlung der Epistaxis – sei sie iatrogen nach Operation oder spontan bzw. aufgrund anderer Grunderkrankung auftretend – spielen Nasentamponaden eine wichtige Rolle. Eine gefürchtete, wenn auch seltene Komplikation ist dabei die Dislokation der Tamponade, wofür in der Literatur letale Verläufe beschrieben sind. Aufgrund dessen herrscht in Deutschland eine große Zurückhaltung gegenüber dem ambulanten Führen nasentamponierter Patienten vor. Material/Methode: Eine Literaturrecherche in den Leitlinien der Fachgesellschaften in Nationen mit vergleichbarem Gesundheitsstandard wurde durchgeführt im Hinblick auf Empfehlungen zum Management von nasentamponierten Patienten. Ergebnisse: Der Blick ins europäische und außereuropäische Ausland offenbart, dass die deutsche Praxis nicht überall geteilt wird. Vielmehr ergibt sich ein deutlich differenzierteres Bild. Nasentamponaden stellen oftmals keinen Hinderungsgrund für eine Entlassung dar. Zudem weisen Studien nach, dass die Evidenz gegen die zwingend stationäre Behandlung nasentamponierter Patienten spricht, sofern wichtige Risikofaktoren bei den Patienten ausgeschlossen wurden. Diskussion: Das in Deutschland übliche Vorgehen, Patienten mit Nasentamponaden nicht nach Hause zu entlassen, findet international wenig Entsprechung. Auf dieser Grundlage wären Studien wünschenswert, um die Patientensicherheit und den Therapieerfolg zwischen ambulant versus stationär geführten Patienten mit Nasentamponaden vergleichend zu ermitteln.Die Autorinnen/Autoren geben an, dass kein Interessenkonflikt besteht.

Overexpression of ACVRL1 inhibits the tube formation ability of HMEC-1 endothelial cells

Introduction: HHT type 2 (HHT2) is an autosomal dominant inherited disease in which one allele of the ACVRL1 gene is mutant. Patients have severe disturbances in angiogenesis and clinically often present with severe nosebleeds and an impaired quality of life. The aim of our study was to investigate how overexpression of ACVRL1 in the endothelial cell line HMEC-1 affects angiogenesis. Material and methods: Overexpression of wild type ACVRL1 was achieved after transfecting a wild type ACVRL1-encoding plasmid into HMEC-1 cells. Quantitative PCR was used to assess gene expression levels. Expression of the ACVRL1 protein was evaluated by fluorescence microscopy. The consequences of ACVRL1 overexpression were evaluated using a Matrigel-based tube formation assay. Tube formation was quantified with the Angiogenesis Analyzer software (ImageJ, Fiji). Results: Significant (p&lt;0.05) ACVRL1 overexpression in HMEC-1 cells was achieved at the nucleic acid level and was confirmed by RT-qPCR. Overexpression of the ACVRL1 protein was demonstrated by immunofluorescence microscopy. Angiogenesis analysis after ACVRL1 overexpression revealed a significantly lower number of vessel junctions, greater segment lengths and more isolated segments. Conclusion/Discussion: Overexpression of wild type ACVRL1 in HMEC-1 cells inhibits their angiogenic ability. It leads to a significant reduction in vessel formation, which is opposite to the enhanced vessel production as seen in HHT2 patients which carry a mutant ACVRL1 allele that gives rise to an inactive ACVRL1 protein. Future studies need to assess if overexpression of wild type ACVRL1 in HHT2 endothelial cells, that are haploinsufficient for ACVRL1, could help to alleviate the symptoms of HHT2 patients by normalizing angiogenesis.Die Autorinnen/Autoren geben an, dass kein Interessenkonflikt besteht.

Visual impairment as the first symptom of metastatic prostate cancer

Visusminderung durch orbitale Metastase als Erstsymptom eines metastasierten Prostatakarzinoms

Introduction Prostate cancer tends to metastasize to the bones, but can also rarely lead to soft tissue metastases, for example in the orbit. These bring with them corresponding symptoms such as double vision or a reduction in visual acuity. We report a case of visual impairment as the first symptom of metastatic prostate carcinoma due to orbital metastasis. Material and methods An 80-year-old male patient presented to the ophthalmology department as an emergency case of a retroorbital mass on the right side with double vision and decreased visual acuity. The patient reported right exophtalmos for one week and double vision since the previous day. The ophthalmologic examination revealed red desaturation and visual acuity of 0.2. There was an exophthalmos on the right eye with preserved motility. CCT and cMRI showed a solid mass at the right lateral orbital rim with extension retrobulbar and into the infratemporal fossa and with invasion via the orbital apex intracranially through the middle cranial fossa. A biopsy was obtained under general anesthesia via an infraorbital approach. Result Histology revealed a low-differentiated adenocarcinoma with immunohistochemical markers as evidence of a prostate carcinoma metastasis. After presentation to the urology department, a tumor board decision was made with a recommendation for antiandrogen therapy and radiotherapy. The patient refused radiotherapy but has responded well to chemical castration so far. The ocular symptoms have currently regressed significantly. Conclusion Ocular symptoms as the first clinical manifestation of prostate carcinoma are very rare. Despite the poor prognosis, prompt and correctly initiated therapy can improve the prognosis and often achieve long intervals of low burden of symptoms.Einleitung Das Prostatakarzinom neigt zur ossären Metastasierung, kann jedoch selten auch zu Weichgewebsmetastasen, zum Beispiel in der Orbita, führen. Diese bringen entsprechende Symptome wie Doppelbilder oder eine Visusminderung mit sich. Wir berichten über den Fall einer Visusminderung als Erstsymptom eines metastasierten Prostatakarzinoms aufgrund einer orbitalen Metastase. Material und Methoden Ein 80-jähriger Patient wird von den Kollegen der Augenklinik notfallmäßig bei V.a. retrobulbäre Raumforderung rechts bei Doppelbildern und Visusminderung vorgestellt. Der Patient berichtete über einen seit einer Woche bestehenden Exopthalmus rechts und Doppelbilder seit dem Vortag. Ophtalomologisch wurde eine Rotfarbentsättigung und ein Visus von 0,2 festgestellt. Es zeigte sich ein Exophtalmus rechts bei erhaltener Motilität. Im cCT und cMRT zeigte sich eine solide Raumforderung am lateralen Orbitarand rechts mit Ausdehnung nach retrobulbär und in die Fossa infratemporalis sowie ein Einbruch über den Apex orbitae nach intrakraniell. Es erfolgte eine Probenentnahme über einen infraorbitalen Zugang. Ergebnis Histologisch zeigte sich ein niedrig differenziertes Adenokarzinom mit immunhistochemischen Markern einer Prostatakarzinommetastase. Nach Vorstellung in der Urologie folgte der Tumorboardbeschluss mit Empfehlung zur antiandrogenen Therapie und Strahlentherapie. Der Patient lehnte die Bestrahlung ab, spricht aber bislang gut auf die chemische Kastration an. Die okulären Symptome haben sich derzeit deutlich zurückgebildet. Schlussfolgerung Okuläre Symptome als erste klinische Erscheinung eines Prostatakarzinoms sind sehr selten. Trotz der schlechten Prognose kann eine zügig und korrekt eingeleitete Therapie diese verbessern und oft lange Symptomfreiheit erreichen.
