Spinal arachnoid cysts and their differentiation from spinal cord herniation – using ultrasound for intraoperative evaluation

Spinal arachnoid cysts (SAC) are benign but space-occupying outpouchings of the arachnoid membrane, which may be congenital or acquired. Adult presentations often include back pain and spinal cord compression symptoms. MR imaging is key for diagnosis, complemented by CT myelography and ultrasound for surgical planning and real-time confirmation. On MR imaging the spinal cord may appear to be attached to the ventral dura and SAC may subsequently be mistaken for ventral myelon herniation (VMH).We performed a single-center retrospective analysis on patients treated surgically for suspected ventral myelonherniation (VMH) or intradural spinal arachnoid cysts (SAC) from 2007 to 2024. Neurological status was assessed pre- and postoperatively, and surgical approaches documented. Intraoperative ultrasound imaging was used in two illustrative cases.Among 44 patients with suspected VMH in 14 patients, initial imaging led to misdiagnosis of VMH in five patients. In nine patients VMH was found intraoperatively, 35 were diagnosed with SAC. In one patient a ventral myelon defect was noted additionally. Most pathologies were thoracic (77%), and clinical improvement was observed in 66% of cases. Intraoperative ultrasound effectively aided diagnosis and surgical guidance in two illustrative cases.SAC are rare lesions causing spinal cord compression, often misdiagnosed as ventral myelon herniation VMH. This study highlights intraoperative ultrasound as a handy tool for accurate diagnosis and surgical planning. Ultrasound enables precise cyst localization, reducing invasive procedures and improving outcomes. Total cyst resection is emphasized to prevent recurrence, though some patients may experience neurological decline due to surgical manipulation potentially by dura inspection ventral of the myelon. The findings stress the importance of ultrasound in enhancing diagnostic and surgical strategies for SAC and suspected VMH.

Dorsal arachnoid webs: Timing of surgical intervention and its impact on patient outcomesDorsale arachnoidale Membranen: Zeitpunkt des chirurgischen Eingriffs und seine Auswirkungen auf die Patientenergebnisse

Dorsal arachnoid webs (AW) are abnormal formations in the upper thoracic spine, often causing spinal cord displacement and symptoms like back pain, sensory deficits, gait disturbances, or remaining asymptomatic. These AW can impede cerebrospinal fluid (CSF) flow, leading to syringomyelia, which may be detected as the "scalped sign" on MRI. Treatment typically involves surgical resection to relieve spinal cord compression. We present three cases that underwent surgery, highlighting the challenges and clinical outcomes over six months. The optimal timing of surgical intervention remains debated, given the rarity of this condition and varied clinical outcomes.A retrospective review of three patients who had surgery for AW, assessing clinical outcomes using the T-JOA score. Pre- and postoperative images were analysed to track changes in spinal cord compression and surgical effectiveness.All three patients had lower back pain and numbness in the lower extremities. Two also had gait instability and one had bladder-bowel dysfunction (BBD). MRI showed the "scalpel sign" in all three patients and syringomyelia in two patients. One patient with syringomyelia was managed conservatively for six years but underwent surgery after worsening of symptoms. The other two patients had surgery early (2–6 weeks after symptom onset). All surgical interventions consisted of fenestration and web resection without instrumentation. Patients who underwent surgery shortly after symptom onset demonstrated substantial improvement in their total T-JOA scores (77.8% and 100% recovery), compared to 40% improvement observed in the patient initially managed conservatively. Numbness in the lower extremities resolved in the two patients who underwent early surgery but persisted in the patient initially managed conservatively. Post-operative improvement in BBD was observed.This case series suggests that early surgical intervention after symptom onset may be associated with better outcomes. Patients who underwent early surgery demonstrated substantial improvement in T-JOA scores and resolution of symptoms like numbness and BBD, compared to delayed surgical intervention following conservative management. These findings underscore the importance of considering early surgical intervention for symptomatic AW, though further research with larger patient cohorts is needed to confirm optimal treatment strategies.

Klinische Ergebnisse nach Resektion spinaler Hämangioblastome bei von Hippel-Lindau Syndrom - Eine retrospektive Analyse eines überregionalen VHL-ZentrumsClinical Outcome after resection of spinal hemangioblastomas in von Hippel-Lindau disease - A retrospective analysis in a nationwide VHL center

Spinale Hämangioblastome (HBL) sind eine seltene Entität und stellen mit ca. 10% einen geringen Anteil der intramedullären Tumoren dar. Beim genetisch bedingten von Hippel-Lindau (VHL) Syndrom kann es zur Ausbildung multipler HBL und somit Häufung dieser Entität kommen.Ziel dieser Arbeit war die Identifikation etwaiger Risikofaktoren für ein schlechtes klinisches Outcome von VHL-Patienten nach Resektion spinaler HBL in einem nationalen VHL-Zentrum, um somit die langfristige Versorgung aufgrund oft multipel notwendiger chirurgischer Eingriffe betroffener Patienten zu verbessern.Dies ist eine retrospektive Evaluation aller in unserem Zentrum operierten VHL-Patienten von 01/2010 bis 12/2023 und dabei Analyse nur isoliert spinaler HBL-Resektionen (subC2 bis sakral). Auch Operationen mit zeitgleicher HBL-Entfernung cerebellär oder am kraniozervikalen Übergang wurden ausgeschlossen. Es wurden unter anderem prä- und postoperative klinische Parameter (Modified Rankin Scale (MRS), Modified McCormick Score (MMS), Ataxie, vegetative Störung), perioperative Befunde (Indikation, Zugang, OP-Dauer, Blutverlust) sowie MRT-Befunde (Tumorlage, Tumor-/Zystenvolumen) analysiert. Zusätzlich erfolgte eine Risikofaktoranalyse für Patienten mit einem verschlechterten postoperativen Ergebnis mittels binärer logistischer Regression.Es wurden 130 Operationen (38,4 ± 13,8 Jahre; 46,2% weiblich) analysiert. Die anatomische Verteilung der Operationen ist in Abb.1 dargestellt. Insgesamt wurden dabei 185 Tumoren entfernt, wobei in 108 (83,1%) der Eingriffe ein Tumor reseziert, in den restlichen Operationen bis zu neun Tumoren (n=1; 0,8%) in einer Sitzung reseziert wurden. Präoperativ waren 30 (23,1%) Patienten asymptomatisch (MRS=0), 39 (30%) Patienten waren bereits höhergradig hilfsbedürftig (MRS≥3). Weniger als 10% der Patienten zeigten eine klinische Verschlechterung im Follow-up (7,7% Zunahme MRS und 9,2% Zunahme MMS, Tab.1). In der Risikofaktoranalyse war für eine klinische Verschlechterung ein Trend für ein größeres Volumen des resezierten soliden Tumoranteils identifizierbar (p=0,098).Es zeigt sich insgesamt ein gutes klinisches Outcome der in manchen Fällen bereits im Rahmen der Grunderkrankung klinisch eingeschränkten vHL-Patienten mit einer langfristigen Verschlechterungsquote von unter 10% nach spinaler HBL-Resektion. Die resezierte Tumormasse scheint dabei einen Risikofaktor darzustellen.

Characteristics of spinal meningiomas determining postoperative outcomes

Spinal intradural tumors, including ependymomas, astrocytomas, meningiomas, and neurinomas, constitute about 15% of central nervous system tumors. Meningiomas, comprising approximately 40% of these, are often linked to edema, calcification, and potential spinal canal compression due to extensive growth. This study evaluated the impact of tumor characteristics, neurological status, and surgical techniques on postoperative outcomes, using changes in the modified McCormick scoreThis monocentric, retrospective cohort study included patients from 2007 to 2024 with histologically confirmed intradural meningiomas, regardless of age, grading, or localization. Inclusion required clinical follow-up, preoperative MRI, and if available CT for calcification assessment. Postoperative outcomes were categorized as worsening, persistence, or improvement of neurological status based on changes in the modified McCormick Score.In total, we analyzed 141 patients with a median age of 67. The median span over segments was 1, tumor volume 0.63cm³ and ratio to spinal canal 0.34 with foremost localization in the thoracic spine. In 18 patients a CT scan was available with 11 patients showing calcifications. Comparing the 3 categories we found statistically significant differences comparing number of affected segments and ratio to the spinal canal specifically comparing worsening to persistence and improvement (p < 0.05, AUC 0.5). Calcification and edema do not impact the postoperative outcome. Median modified McCormick scores pre- and post-surgery were II and I, respectively.In summary, tumor volume, the tumor-to-spinal canal volume ratio, and the number of affected spinal segments emerged as the primary prognostic factors for postoperative outcome in spinal meningiomas. In contrast, calcification and edema appeared to have a minor impact. Surgical resection typically resulted in improved neurology.

Clinical and automated volumetric analysis reveal predictors of unfavorable outcome after spinal meningioma resection

While functional outcomes after spinal meningioma (SM) resection are generally favorable, some patients experience persistent neurological deficits that reduce quality of life. This study aimed to identify factors influencing full neurological recovery in a large patient cohort.We present a single-center retrospective analysis of consecutive patients undergoing surgery on spinal meningiomas between 2007 and 2022. Clinical surgical data were integrated with MRI-based automated volumetric analyses of preoperative tumor burden. Patients with a favorable outcome (Frankel grade E) at final follow-up were compared to patients with patients with non-resolving neurological symptoms (Frankel A to D) patients.A total of 189 patients with a histologically diagnosed WHO grade 1 meningioma, including 40 males and 149 females (79%) with a median age 64 were included in the analysis. Meningiomas were mostly found in the thoracic (n=123, 65%) followed by the cervical (n=41, 22%) spine.Upon admission, motor deficits or gait deficits (Frankel A-D) were present in 93 patients (49%). Gross total resection was achieved in 182 patients (96%), with a surgical complication rate of 5% (n= 9, requiring revision).). After a mean follow-up of 860±470 days, 67% of patients recovered fully (Frankel E). Comparison of patients demonstrating full recovery (Frankel E) versus patients with non-resolving neurological deficits revealed an overrepresentation of advanced age (60 years), the presence of ataxia, sensory or motor deficits, with permanent neurological deficits at final follow-up in a univariate analysis (p<0.001). No significant differences were noticed in tumor volume between both groups (mean= 2.31 vs 2.49 ml, p= 0.651). In a multivariate analysis, only age and Ki-67 >5% were associated with non-resolving deficits at final follow-up, when adjusting for tumor volume in a further multivariate sub-analysis of 88 patients, only a pre-operative Frankel grade of E was associated with a favorable outcome.In this large, volumetry-informed series of patients with spinal meningioma, high age and a low-proliferation index, along with pre-operative neurological deficits constituted a higher risk of non-resolving neurological deficits after resection.

Minimal-invasiver Ansatz zur Resektion von Cauda-Ependymomen: Eine vergleichende Studie

Die primäre Behandlung von Cauda-Ependymomen besteht in einer vollständigen und sicheren Resektion. Während einige Autoren eine en-bloc-Resektion mittels Laminektomie empfehlen, um eine vollständige Tumorresektion (GTR) zu erreichen und das Risiko einer Tumoraussaat in der Zerebrospinalflüssigkeit (CSF) zu minimieren, andere befürworten minimal-invasiver Techniken, die potenzielle Vorteile bieten können. Diese Studie zielt darauf ab, das Ausmaß der Resektion (EOR) und die Tumorrezidivraten zwischen den beiden Ansätzen zu vergleichen.Wir analysierten retrospektiv Daten aus dem Europäischen Multizenter-Register für juxtamedulläre Tumoren (EURJMT), das Operationen zwischen 2010 und 2022 an sieben europäischen Zentren umfasst. Wir identifizierten Fälle von Cauda-Ependymomen und unterteilten die Patienten in zwei Kohorten. Die erste Kohorte bestand aus Patienten, die einem mittleren Zugang (Laminektomie oder Laminoplastik) unterzogen wurden, während die zweite Kohorte eine einseitige Laminotomie oder Hemilaminektomie durchlief. Große Tumoren, die mehr als drei Wirbelsäulensegmente umfassten, wurden von der Analyse ausgeschlossen.Insgesamt wurden 53 Verfahren analysiert. Von diesen wurden 28 mittels Hemilaminektomie durchgeführt, wobei in 26 Fällen (92,9 %) eine GTR erreicht wurde. In der Laminektomie-Kohorte (n = 25) wurde in 22 Fällen (88,0 %) eine GTR erzielt (P = 0,55). Ein Tumorrezidiv trat bei vier Patienten nach Laminektomie und bei zwei Patienten nach Hemilaminektomie auf (P = 0,31). Die Dauer des Krankenhausaufenthalts (LOS) (P = 0,69) und die Operationsdauer (P = 0,19) unterschieden sich zwischen den Gruppen nicht signifikant. Allerdings war das progressionsfreie Überleben (PFS) in der Hemilaminektomie-Kohorte signifikant besser, mit einer durchschnittlichen PFS von 45 Monaten im Vergleich zu 25 Monaten in der Laminektomie-Kohorte (P = 0,008).Unsere Ergebnisse zeigen, dass die stückweise Resektion von Cauda-Ependymomen mittels eines minimal-invasiven Ansatzes hinsichtlich des Resektionsausmaßes und der Rezidivraten nicht unterlegen ist. Darüber hinaus bietet der minimal-invasive Ansatz einen signifikanten Vorteil in Bezug auf das progressionsfreie Überleben und kann eine schnellere Genesung ermöglichen.
