Osteoporose und Sarkopenie sind altersbedingte Erkrankungen mit steigender Inzidenz. Ziel ist die Untersuchung eines Zusammenhangs zwischen Sarkopenie der beckenüberspannenden Muskulatur, Osteoporose und anthropometrischen Daten.In einer retrospektiven Studie wurden Patientendaten im Zeitraum von 04/2021 bis 05/2023 analysiert. Eingeschlossen wurden alle Fälle, bei denen sowohl eine MRT des Beckens inkl. mDIXON Fat quant Sequenz (mDix) als auch eine CT der LWS vorlagen. In der mDix wurden zur Beurteilung der Sarkopenie Atrophiekriterien wie Querschnittsfläche und der Fettgehalt (FF-Wert) des Musculus erector spinae (MES), psoas (MPS), gluteus maximus (MGM) et medius (MGm) und iliacus (MIL) bestimmt. Die Messung der individuellen Knochenqualität erfolgte anhand der Hounsfield Units (HU) im 5. Lendenwirbel mit folgender Gruppenzuordnung der Patienten: HU >100 – knochengesund, HU ≤100 – Osteoporose.Der Gruppe HU ≤100 wurden 69 Patienten zugewiesen (81 ± 8 Jahre, m: 6, w: 63), 31 der Gruppe >100 HU (65 ± 20 Jahre, m: 21, w: 10). In der statistischen Analyse zeigte sich zunächst ein mittelbarer Zusammenhang zwischen Sarkopenie und Osteoporose, wobei Gruppe HU≤100 MPS und MIL signifikant niedrigere Querschnitte gegenüber HU >100 aufwiesen. MGM wies hingegen größere Werte auf. Keine signifikanten Unterschiede zeigten sich für MES und MGm. Hinsichtlich des Fettgehalts im MES zeigten HU ≤100 Patienten höhere Werte. Die Betrachtung anthropometrischer Variablen ergab allerdings einen signifikanten Einfluss von Geschlecht (p=<0,001), Alter (p=<0,001), Größe (p=<0,001) und Gewicht (p=0,002) auf den Zusammenhang von Sarkopenie und Osteoporose. Diese beeinflussten Querschnitte und Fettgehalt heterogen, wobei das Gewicht den stärksten Einfluss hatte. Nach Adjustierung der Daten anhand der Kovariablen zeigten sich für Muskelquerschnittsflächen keine Unterschiede mehr zwischen den Gruppen. Jedoch wiesen osteoporotische Patienten wider Erwarten zum Teil sogar niedrigere FF-Werte auf.Es besteht ein indirekter Zusammenhang zwischen Knochenqualität und Sarkopenie, der primär durch Alter, Geschlecht, Größe und Gewicht beeinflusst wird. Direkt scheint keine Wechselwirkung vorzuliegen; vielmehr fördern Alter, weibliches Geschlecht und körperliche Konstitution beide Zustände unabhängig voneinander.

Impact of Fracture Morphology and Bisegmental Cobb Angle on Treatment Decisions in Osteoporotic Thoracolumbar Vertebral Fractures

Osteoporotic vertebral fractures (OVCF) are assessed using the bisegmental Cobb angle, guiding treatment decisions from conservative therapy to surgical interventions. The OF-classification (kappa = 0.63) stratifies fractures by severity and informs therapy, but its relationship with Cobb angles and outcomes remains unclear. This study investigates their impact on treatment decisions and outcomes.This analysis includes thoracolumbar OVCF (Th11-L2) patients from the EOFTT cohort. OF1 and OF5 fractures were excluded. The Cobb angle, measured on CT scans at the day of treatment decision (DTD), was categorized by a -15° kyphotic threshold. Fractures were classified (OF2–OF4), and treatments categorized as conservative, augmentation, or instrumentation. Statistical methods included general linear models, multinomial logistic regression, and ROC analyses to evaluate relationships between Cobb angle, OF-classification, and treatments.Among 350 patients (mean age: 74±10 years), treatments included 120 conservative, 83 augmentation, and 147 instrumentation. Bone quality, fracture localization, and age were similar across groups. Higher OF-classifications and pain scores were linked to surgical treatments (p<0.001). OF4 fractures showed greater kyphotic misalignment than OF2 (-13±14° vs. -6±13°, p=0.004). The Cobb angle showed no significant group differences (p=0.346) and limited predictive value for treatment (AUC=0.582, p=0.021). The OF-Score was more predictive (AUC=0.697, p<0.001), differentiating augmentation from instrumentation. Multinomial regression confirmed that pain and OF-classifications significantly influenced treatments (p<0.001), while the Cobb angle did not (p=0.313).The Cobb angle has limited predictive value for treatment in OVCF. While it assesses kyphotic misalignment, the OF-classification and pain better predict therapy, especially for distinguishing augmentation from instrumentation. Higher OF-classifications were associated with surgical treatments, aligning with guidelines. Incorporating fracture severity and symptoms beyond imaging could improve treatment algorithms and outcomes in OVCF.

Influence of different treatment methods on the sagittal profile and clinical outcome of patients with osteoporotic fractures of the spine (OVCF) at the thoracolumbar junction (TLJ)

With an aging population and therefore an increase in OVCF, the question of the ideal treatment arises. The aim of a surgical treatment therefore is to stabilize or reverse the decline of the vertebrae and with that to sufficiently reduce pain and regain mobility. This study objective is to evaluate the effect of the chosen therapy, both conservative and surgical, on the sagittal profile as well as the clinical outcome of OVCF.Patient data from the Evaluation of the Osteoporotic Fracture Classification (OF), Treatment Score and Therapy Recommendations (EOFTT) Study were used. On the day of treatment fracture morphology according to the OF-Classification were recorded. The treatment options - conservative, augmentation techniques (kypho- or vertebroplasty) and surgical instrumentation (bi-/multisegmental) were compared radiologically using the bisegmental Cobb angle biCA. Therefore, three defined time intervals were evaluated: the Day of Treatment Decision DTD to Discharge DIS, DIS to FU, and DTD to FU. The clinical outcome measures used were the ODI, the VAS, the EQ-5D-5L and the TuG Test.Fracture types were OF3 (44%), OF4 (28%) and OF2 (23%). 33% were treated conservatively, 67% underwent surgery. 23% received an augmentation, 44% an instrumentation. Surgical interventions showed a significant improvement of biCA from the DTD to DIS (augmentation p=0.002; instrumentation p < 0.001). The biCA reduced from DIS to FU across all groups (p= 0.685). Patients have a significant improvement of biCA after instrumentation comparing DTD to FU, after augmentation patients revealed a subsidence to the values at DTD in biCA. The ODI and VAS showed an improvement from the DTD to DIS and again to FU. The TuG showed a significant improvement, particularly in conservatively treated and instrumented patients at FU.The finding that patients with posterior fixation compared to the augmented group showed the least functional restriction, the lowest VAS scores, shorter TuG times and better EQ5D results at FU could be interpreted as an effect of the sustained correction of fracture related kyphosis.The long-term outcome of augmentation procedures raises questions about the indication for these surgical techniques and suggests a preference for instrumentation.The sagittal profile should be included initial to better understand the effect of the profile correction and to initiate prospective studies comparing augmentation vs. fixation.

Korrelation zwischen dem Gesamtvolumen von Hyperintensitäten der weißen Gehirnsubstanz und der Knochendichte im Bereich des thorakolumbalen Übergangs der Wirbelsäule

Die Osteoporose ist eine Volkskrankheit, die durch ein Missverhältnis der Aktivität in knochenauf- und abbauenden Zellen gekennzeichnet ist. Die Verschlechterung der Mikroarchitektur geht mit einer verminderten Knochendichte einher, wodurch das Frakturrisiko steigt. In bisherigen Studien wurde bereits ein Zusammenhang zwischen Entzündungsprozessen, die durch die immunmodulatorischen Eigenschaften des Vitamin D beeinflusst werden, und der Pathogenese von Osteoporose beschrieben. Kürzlich wurde die Korrelation zwischen dem Vitamin D Spiegel im Serum und der Entwicklung von Hyperintensitäten im Bereich der weißen Gehirnsubstanz ("White Matter Hyperintensities") analysiert. Hierbei zeigte sich, dass niedrige Vitamin D Spiegel mit einer vermehrten Entwicklung von WMH 10 Jahre später einhergehen. WMH sind ein Risikofaktor für die Entstehung von kognitiven und funktionalen Beeinträchtigungen. Ebenfalls konnte in Studien eine negative Korrelation zwischen Vitamin D3 Spiegeln und der Entstehung von WMH gezeigt werden.In dieser Studie wurde der Zusammenhang zwischen dem Gesamtvolumen von WMH und der Knochendichte im Bereich des thorakolumbalen Übergangs der Wirbelsäule untersucht, um Erkenntnisse in Bezug auf die Pathogenese der Osteoporose zu erzielen.Die zu untersuchenden Parameter wurden im Rahmen einer populationsbasierten Studie aus EBCTs der thorakolumbalen Wirbelsäule und kranialer MRT-Bildgebung erhoben. In der vorliegenden Studie wurden Daten von N = 530 ProbandInnen analysiert. Die Erfassung der Knochenqualität erfolgte mittels Bestimmung der Hounsfield Units in den EBCTs und die der WMH über die Volumetrie in cMRTs. Für die Studie liegt ein positives Ethikvotum vor.Die Analyse liefert eine signifikante, negative Korrelation zwischen WMH und der gemessenen Knochendichte (r = -0.21, p < 0.001).Das bisherige Ergebnis könnte bereits erste Hinweise auf einen Zusammenhang zwischen der Pathogenese der Osteoporose und der Entstehung von WMH geben, sodass durch zusätzliche Studien Ansätze für Prävention sowie für frühzeitige Therapieoptionen der Osteoporose entwickelt werden könnten.

Sarcopenia in patients with osteoporotic vertebral fracture is associated with higher risk of multiple fractures

Introduction:The association of sarcopenia and increased fracture risk has been well documented. However, studies investigating the role of sarcopenia in osteoporotic vertebral fractures (OVFs) are scarce.Objectives:This study aimed to retrospectively evaluate the prevalence of sarcopenia among patients with OVFs and its association with clinical presentation and outcomes.A retrospective analysis was conducted on patients with OVFs treated at a single spinal center between January 2022 and June 2024. Quantitative assessment of the psoas muscle was performed using axial spinal CT images. The cross-sectional area of the psoas muscle (pCSA) was measured at the L4 vertebral level and normalized using the psoas muscle index (PMI = pCSA/height²) and the psoas muscle lumbar vertebral body index (PLVI = pCSA/vertebral body area). Sarcopenia was defined using established PMI cut-off values. Clinical parameters, osteoporosis diagnostics (DEXA T-scores, CT-based Hounsfield Units [HU]), and clinical outcomes were recorded.A total of 142 patients with OVFs were included, with a median age of 81 (74 – 85) years and 94 (66.2%) females. Sarcopenia was identified in 103 (72.5%) patients. Patients with sarcopenia had significantly lower BMI, lower serum Albumin and lower lumbar T-scores (Fig. 1) in DEXA scans. Multiple (2 or more) fractures were significantly more frequent in patients with sarcopenia (37.2% vs 18%, p = 0.018, see Fig. 3). Logistic regression revealed that patients with sarcopenia were 2.78 times more likely to have multiple fractures (95% CI; 1.1 – 6.9, p = 0.027). No significant differences were observed in HU values (Fig. 2), time to mobilization postoperative, length of hospital stay, or incidence of postoperative wound infection rates.Sarcopenia is highly prevalent among patients with OVFs and is a significant risk factor for multiple acute fractures. Assessing the validity of the OF-Score in Treating Osteoporotic Spine Fractures: Initial Insights from a Single-Center Retrospective Analysis

Osteoporotic spine fractures can be managed conservatively or surgically. The OF score was introduced to aid clinical decision-making by integrating multiple factors, including fracture morphology, osteoporosis severity, deformity progression, pain intensity, neurological symptoms, mobility, and overall health status. This study aims to assess the validity and feasibility of the OF score as a standardized tool for guiding treatment in osteoporotic spine fractures.We retrospectively analyzed 177 patients (mean age: 77 years; female-to-male ratio: 2.32:1) with osteoporotic spine fractures treated at our neurosurgical department between January 2023 and December 2024. A total of 264 fractures were evaluated, of which 219 received surgical treatment and 45 were managed conservatively. The OF score was calculated for each fracture.Among the 219 surgically treated fractures, the OF score recommended surgery in 47.48% (n=104). In 11.8% (n=26), both treatment options were suggested by the OF score. However, 40.6% (n=89) were operated on despite a conservative recommendation. Within this subset, OF2 fractures were the most frequent (84%, n=75), whereas OF3 fractures were less common (14.6%, n=13). kyphoplasty was performed in 87.6% of these cases, with the remainder treated via dorsal instrumentation. Among the 45 fractures managed conservatively, the OF score recommended conservative treatment in 86.6%, whereas 8.8% were managed conservatively despite a surgical recommendation. In 4.4%, both approaches were indicated.The OF score is a valuable tool for guiding the management of osteoporotic spine fractures. However, factors such as pain intensity and diminished quality of life can significantly influence final treatment decisions—especially when conservative measures (e.g., pain therapy including strong opioids, physiotherapy, and orthotic support) fail to provide relief. Certain parameters of the OF score may eventually need revision, particularly when they cannot be accurately assessed at the time of clinical decision-making (e.g., ongoing fractures). On the other hand, minimally invasive surgery (e.g., kyphoplasty or percutaneous instrumentation) may offer with relative low-risk a rapid improvement of symptoms. With further refinement, the OF score holds promise as a reliable, standardized instrument for optimizing treatment strategies in patients with osteoporotic spine fractures.

Beziehung zwischen DEXA-basierten T-Scores von Lendenwirbelsäule, Hüfte und Femur und CT-basierten Hounsfield-Einheiten bei Patienten mit manifester Osteoporose

Die Dual-Energy-Röntgenabsorptiometrie (DEXA) und der daraus resultierende T-Score gelten derzeit noch als der Goldstandard zur Knochendichtemessung und Diagnose einer Osteoporose. In dieser Studie untersuchten wir die Beziehung DEXA-basierter T-Scores der Lendenwirbelsäule (LWS), Hüfte und Femur im Vergleich zu CT-Hounsfield Units (HU) bei Patienten mit manifester Osteoporose.Von 2019 bis 2024 untersuchten wir 350 Patienten mit osteoporotischen Frakturen der Wirbelsäule. Alle Patienten erhielten eine CT-Bildgebung der LWS. 170 Patienten wurden via DEXA-Scan untersucht (n=170 mit T-Score der LWS, n=123 mit T-Score der Hüfte, n=113 mit T-Score des Femurs). Aus den Patientendaten wurden DEXA- und CT-Scans herausgefiltert und nur Patienten eingeschlossen, bei denen beide Untersuchungen vollständig durchgeführt wurden. Der DEXA-Scan wurde anhand der T-Scores von LWS, Hüfte und Femur bewertet. Die HU wurde axial in jeweils drei nicht frakturierten Wirbelkörpern der Lendenwirbelsäule bewertet. Für jeden Wirbelkörper wurde eine ROI (Region of Interest) unterhalb der Deckplatte, in der Mittelplatte und oberhalb der Grundplatte in der Spongiosa gemessen. Der HU-Cut-off-Wert für eine osteoporotische Fraktur wurde auf Grundlage des neuen modifizierten AO-Scores für osteoporotische Frakturen aufBei 113 Patienten lagen vollständige Untersuchungen von DEXA- und CT-Scans vor. Das Durchschnittsalter betrug 80 Jahre, 68% der Patienten waren weiblich, 22% waren männlich. Die meisten Frakturen im Thorakolumbalen Übergang (19%). Bei 165 Patienten (97%) der Patienten wurde ein niedriger HU-Wert festgestellt, wobei in 33 % der DEXA-LWS-Scans, 26,8 % der DEXA-Femur-Scans und 24,1 % der DEXA-Hüft-Scans ein T-Score im osteoporotischen Bereich festgestellt wurde. Es wurde ein statistisch signifikanter Zusammenhang zwischen HU und DEXA-T-Score der Wirbelsäule (11,6 (2, n= 113), p = 0,003) sowie DEXA-T-Score der Hüfte (10,3(2, n= 113, p = 0,006) festgestellt. Es wurde kein signifikanter Zusammenhang zwischen dem DEXA-T-Score des Oberschenkels und der HU festgestellt. Darüber hinaus wurden keine weiteren signifikanten Unterschiede in Bezug auf Demografie, Alter, Anzahl der Frakturen und Ort der Fraktur festgestellt. Patienten, die eine Osteoporose im DEXA-Scan aufwiesen, zeigten alle sehr niedrige CT-HU von unter 50.Auch wenn der DEXA-Scan noch immer der Goldstandard ist, so scheint die HU besser geeignet, eine manifeste Osteoporose zu detektieren. Nur bei Patienten mit HU-Werten unter 50 konnte DEXA zuverlässig eine Osteoporose nachweisen. Im Vergleich scheinen die T-Scores der LWS und Hüfte besser geeignet, eine Osteoporose nachzuweisen, als der T-Score von Femur. Jedoch ist die Reliabilität auch der T-Scores für die LWS im Vergleich zur Reliabilität der CT-HU der LWS gering und wahrscheinlich ungeeignet, eine manifeste Osteoporose festzustellen.
