Armdrücken ist v.a. unter jungen Männern populär.1 Allerdings ist Armdrücken auch die führende Ursache von Humerusfrakturen in speziellen Populationen, beispielsweise bei jungen Soldaten.1 Spiralfrakturen des Humerusschaftes sind die mit Abstand häufigsten Verletzungen verursacht durch Armdrücken. Hierbei ist besonders der N. radialis im Sulcus nervi radialis des Humerus gefährdet. In dieser monozentrischen Fallserie soll das langfristige Ergebnis der Nervenfunktion nach konservativer bzw. chirurgischer Therapie erhoben und in die Literatur eingegliedert werden.Es handelt sich um eine Datenbankrecherche eines universitären Neurozentrums im Hinblick auf Patienten, die zwischen 2012 und 2024 aufgrund einer durch Armdrücken verursachten N. radialis Parese neurologisch oder neurochirurgisch behandelt wurden. Zusätzlich erfolgte eine telefonische Nachbefragung zum Langzeit-Outcome. Die Ergebnisse sowie potenzielle Risikofaktoren wurden mit der bisherigen Literatur abgeglichen und analysiert.Insgesamt konnten vier Patienten identifiziert werden. Alle waren männlich, der Altersdurchschnitt bei Trauma betrug 23,3 Jahre (Range 20-29 Jahre). Zwei Patienten erlitten direkt nach dem Unfall eine vollständige, distale Radialisparese, bei den anderen beiden bestand initial eine inkomplette Lähmung. Bei drei der vier Patienten wurde aufgrund der hochgradigen anhaltenden Funktionsstörung eine Neurolyse des N. radialis in unserer Klinik durchgeführt. Postoperativ war bei zwei Patienten innerhalb weniger Wochen das Ansteuern der Hand- und Fingerextension wieder möglich. Der dritte Patient erlangte nach etwa 4 Monaten die vollständige Nervenfunktion zurück. Eine Erhebung des Langzeitverlaufs des vierten, konservativ geführten, Patienten war aufgrund fehlender Erreichbarkeit nicht möglich.Spiralbrüche des distalen Humerusschafts stellen eine häufige Verletzung beim Armdrücken dar. Eine assoziierte Radialisparese ist sowohl durch den Unfallmechanismus als auch die operative Versorgung dislozierter Frakturen möglich. Die frühzeitige nervenchirurgische Intervention kann bei persistierender höhergradiger Funktionsstörung sinnvoll sein und zum Wiedererlangen der vollständigen Nervenfunktion führen. 1Moloney DP, Feeley I, Hughes AJ, Merghani K, Sheehan E, Kennedy M. Wir berichten über einen 35-jährigen Patienten, welcher plötzlich eine komplette, schmerzhafte N. radialis-Plegie erlitt. Es bestand initial der Verdacht auf einen Myokardinfarkt, welcher ausgeschlossen werden konnte. Erst 3 Monate später wird ein Neurologe konsultiert welcher aufgrund der Elektrophysiologie und Nervensonographie den Verdacht auf eine Nerventorsion des N. radialis stellt.Es erfolgte die operative nervenchirurigsch-mikrochirurgische Re-torsion des N. radialis unter intraopeativen Monitoring mit eindrucksvollem intraoperativen Befund. Im weiteren Verlauf zeigte sich nach einem Jahr unter Muskelstimulation, Physio- bzw. Ergotherapie eine komplette Regeneration des N. radialis.Torsionsneuropathien sind sehr seltene Pathologien, welche an allen peripheren Nerven auftreten können, vor allem an der oberen Extremität und am häufigsten ist der N. radialis betroffen.Dieser Fall zeigt, dass die Kenntnis von seltenen Nervenkompressionssyndromen essentiell wichtig ist um gut therapierbare periphere Nervenpathologien frühzeitig zu diagnostizieren und therapieren zu können. Auch schon das Wissen um diese seltenen Nervenpathologien kann dem Patienten den richtigen Therapieweg aufzeigen. Leider werden immer noch zu viele Patienten zu spät der adäquaten Therapie zugeführt. Somit ist es unsere Aufgabe als Nervenchirurgen seltene Pathologien unter den Kollegen bekannter zu machen und ein Netzwerk zur zeitnahen Versorgung solcher Patienten zu etablieren.

Case report: Epitheloid Hemangioendothelioma mimicking a Schwannoma of the medial antebrachial cutaneous nerve

Epitheloid Hemangioendotheliomas (EHE) are rare vascular tumorous lesions with potential malignant transformation. Only a few cases have been reported. Sometimes, they can mimic other nerve sheath tumours, and it is difficult to differentiate between different tumour entities using imaging alone. A biopsy is often required to confirm the diagnosis. They can be multifocal, with lung, liver, and bone metastases.We present the case of a 67-year-old woman with a history of persistent left upper arm pain since 2020 without accompanying neurological deficits. Several MRIs revealed unusual imaging characteristics over the years, including heterogeneous contrast enhancement on T1- and T2-weighted sequences and peritumoral haemorrhage. An FDG-PET scan demonstrated significant tumour progression (dimensions: 19 × 18 mm, SUV mean/max: 4/7.1), raising suspicion of a malignant process rather than a benign nerve sheath tumour.Due to the patient´s ongoing pain and the need for a definitive diagnosis, microsurgical resection was indicated. Intraoperatively, the tumour was found to encase and infiltrate the medial antebrachial cutaneous nerve. Apart from a small remnant along the vessels, the lesion was completely resected, and the neural structure was preserved. Histopathological examination identified the WWTR1-CAMTA1 fusion gene, confirming a diagnosis of epithelioid hemangioendothelioma.Epitheloid hemangioendothelioma may mimic benign nerve sheath tumours (e.g., schwannoma). It is difficult to differentiate these entities from other tumours based on imaging, and a biopsy should confirm the diagnosis. The experience from this case demonstrates that nerve-preserving resection is possible without clinical deterioration. Further follow-ups, including MRI, showed a significant reduction in pain and no progression. There was no need for adjuvant therapy.

Epitheloid Hemangioendothelioma mimicking a Schwannoma of the medial antebrachial cutaneous nerve

Case Report: Failure of the peroneal nerve after corrective osteotomy of the left knee

There are several causes of peroneal nerve failure. Its exposed location at the head of the fibula makes it vulnerable to mechanical pressure injuries. Individuals with a slender physique and minimal muscle or fat tissue are significantly more prone to such lesions. Iatrogenic nerve damage can occur due to accidental transection during surgery, pressure from bandages, or traction during tibial osteotomy. Painful peroneal nerve palsy may result from ganglion cysts arising from the proximal tibiofibular joint. Additional causes of compression damage to the peroneal nerve include benign nerve tumors such as schwannomas and neurofibromas. Unlike schwannomas, neurofibromas infiltrate the fascicles of the affected nerve, resulting in a higher risk of nerve injury during surgical removal.An 18-year-old female was presented who underwent an osteotomy to correct a deformity of the left knee in July 2024. Postoperatively, she experienced a distal emphasis of anesthesia in the sensory branch of the peroneal nerve, along with paralysis of the muscles innervated by this nerve. The patient had a slender build, measuring 158 cm in height and weighing 39 kg. Further diagnostic neurosonography revealed a focal, hypoechoic, non-vascularized mass approximately 20 mm long and 5.2 mm wide, located at the level of the distal popliteal fossa. The continuity of the epineurium was preserved, with displacement of the intact nerve fascicles. Externally, the suspected diagnosis was a neuroma in continuity resulting from nerve traction damage related to the previous osteotomy. A benign nerve tumor was also considered in the differential diagnosis. Based on the clinical and imaging findings, we recommended resection of the intraneural mass.Pre- and intraoperative ultrasound, along with fluorescein, were utilized to assist in the microsurgical resection of the intraneural mass. Histopathological analysis confirmed a diagnosis of WHO grade I neurofibroma. Throughout the inpatient stay, there was no observed improvement in the focal neurological deficits postoperatively. A follow-up appointment for clinical evaluation, electrophysiological testing, and imaging is scheduled for February 2025.This case report highlights the complete failure of the common peroneal nerve in the context of a pre-existing neurofibroma. The corrective osteotomy likely resulted in additional traction injury to the nerve. Therefore, assessing the prognosis in this situation is challenging.

Case report: Hibernoma, a rare case of a benign soft tissue tumor on the proximal upper arm

Hibernomas are rare soft tissue tumors primarily composed of brown adipose tissue, believed to originate from remnants of fetal brown fat. The presence of varying amounts of white fat cells, spindle cells, and myxoid stroma leads to a high degree of heterogeneity in MR imaging, making histologic assessment essential for a definitive diagnosis. Since their initial description by Merkel in 1906, over 500 cases of hibernomas have been documented, with a higher prevalence in men compared to women. Commonly reported locations for these tumors include the upper and lower extremities, the neck, and the periscapular region. Clinically, hibernomas typically exhibit slow growth, and symptoms such as pain or neurological and vascular deficits often result from local compression of adjacent structures.A 45-year-old female patient presented as an outpatient with pain radiating from the medial aspect of her left upper arm to the mid-flexor region of her forearm, which had persisted for a year. She also reported a palpable mass in the medial upper arm, as well as mild hypesthesia in the first three digits of her left hand. No manifest paresis was observed. Additionally, the patient had noticed an asymptomatic mass in the left anterior thigh for the past two years. Further MRI diagnostics revealed a 7x2x5 cm T1 hyperintense fat-equivalent mass surrounding the neurovascular bundle between the median and ulnar nerves on the left side, with no significant contrast enhancement. Suspicious lymph nodes were not identified. Based on the clinical presentation and imaging findings, a suspected diagnosis of lipoma was made.We indicated the resection of the lipoma-suspicious mass via an anteromedial approach on the upper arm, utilizing pre- and intraoperative sonographic guidance. The complete resection was successful, with preservation of the neurovascular bundle. Postoperative MRI revealed no evidence of residual tumor. The postoperative course was uneventful, and a follow-up for imaging and clinical evaluation is scheduled for three months post-surgery. The final histopathological report diagnosed a hibernoma, with no signs of malignancy.This case study presents the diagnosis of a very rare hibernoma confirmed on the left upper arm, with a likely second occurrence in the left thigh. The literature indicates that marginal resection is the treatment of choice, and recurrence is not reported after complete resection.
