We present the case of a 72-year-old women who presented 10 years ago with a calcified mass in the apex oft he left ventricle, that was initially diagnosed as organized thrombus and treated accordingly. Over the course of 10 years, the mass showed slow growth and increasing hypertrophy of the adjacent myocardium which raised suspicion and caused reconsideration of the initial diagnosis. The recent availability of Photon Counting Detector CT with its increased spatial and temporal resolution substantially aided in the morphologic characterization of the mass as CAT.

Notice involvement of the insertion of the mitral tendinous chords (H), the patient presented with increasing Mitral valve insufficiency.
