Treatment of peripheral nerve lesions using Gore-Tex® vascular prostheses: An exemplary case from the central hospital of the German Armed Forces Koblenz

Abstract

Case history: 51yo male, fall from 1.6m onto right elbow 01/23 with dislocated elbow fracture and traumatic neurapraxia of the ulnar nerve. Initial treatment via osteosynthesis of the medial epicondyle, capsule reconstruction, neurolysis of the ulnar nerve and external cross-joint fixation. Due to persistent luxation renewing of the osteosynthesis and extended neurolysis. Removing of the external fixator 01/24. clinical findings ulnar paresisInvestigation/Results No ulnar signals in electrophysiology 01/24. Despite intensive physical/occupational therapy patient developed a CRPS. Diagnosis Lesion of the ulnar nerve after traumatic elbow luxationTherapy and Progressions After analgetic optimization complex neurolysis of the ulnar nerve with subsequent wrapping in Gore-Tex® vascular prosthesis and partial implant removal 07/24. Further rehabilitation could be completed promptly. Here the Budapest criteria were regressed and there was no evidence of ongoing pressure/tensile stress on the nerve. According to the last neurological examination 09/24 an improvement in ulnar paresis can be expected through nerve regeneration.Comments Peripheral nerve injuries have high medical and health economic relevance. In polytrauma patients the frequency and severity increases with higher overall injury severity. War-related nerve injuries are of particular relevance to military medicine. Peripheral nerve lesions should be treated as soon as possible. Accompanying osseous, vascular or soft tissue injuries sometimes lead to delayed treatment up to 2 years after injury. Prophylactic wrapping of a neurolyzed peripheral nerve in a vascular prosthesis can improve the postoperative outcome.
