Abstract

Pelvic ring fractures in the young patient are usually the result of a high energy trauma. These injuries are associated with polytrauma and impaired quality of life. Recently the association of pelvic ring fractures and erectile dysfunction has been described. This study assessed the rate and the risk factors for erectile dysfunction following pelvic ring fractures

Cross sectional survey of male patients treated at our Level 1 trauma center for pelvic ring fractures between 2015 and 2020. Assessment of quality of life utilizing the short form 12 (SF-12), and assessment of erectile dysfunction utilizing The International Index of Erectile Function (IIEF-5). Classification of pelvic ring fractures according to Young and Burgess.

Inclusion of 91 patients, aged 53.9 (SD 16.8) years. 76.7% of patients were active smoker, one patient suffers from peripheral vascular disease, 11.7% of patients had arterial hypertension and 7% of patients hypercholesterinemia. 28.3% of patients reported an overall good general health (8/10 points). In total, 15 patients (16.5%) of patients suffer from severe ED (IIEF-5 0-8 points). APC-fractures have a 10 times increased risk of suffering from ED when compared with LC-type of pelvic ring fractures (OR. 9.4, 95%CI 0.5 to 17.35, p = 0.048).

Erectile dysfunction is a relevant complication that affected 16.5% of patients with pelvic ring fracutres. Especially patients who suffer from an APC type of injury, the risk of erectile dysfunction is increased substantially. Awareness and early consultation is advised in patients with APC type of pelvic ring fractures.
