Charakterisierung von PatientInnen mit hereditärer motorisch-sensorischer Neuropathie (HMSN)/Charcot-Marie-Tooth Erkrankung (CMT) – CMT-Registerstudie der Charité Berlin

Abstract-Text (inkl. Referenzen) Hintergrund: Die CMT ist eine unheilbare, chronisch progrediente Erkrankung der peripheren Nerven. In der Entwicklung neuer Therapieansätze sind die genaue Patientencharakterisierung und Biomarkersuche unabdingbar. Da die Verteilung von spezialisierten Zentren für die CMT besonders auf den Süden Deutschlands konzentriert ist, spielt dieses Register für die Anbindung von PatientInnen aus dem Nord-Osten Deutschlands eine große Rolle. Methoden: An der Charité Berlin haben wir seit Anfang 2022 PatientInnen mit CMT in einem Register gesammelt. Hierin erfolgte die Erhebung etablierter klinischer Scores, die Etablierung einer Biobank inklusive Fibroblastenkultur, sowie laborchemische und elektrophysiologische Untersuchungen. Ergebnisse: Seit Anfang 2022 wurden über 40 PatientInnen im Rahmen des CMT-Registers rekrutiert. Bei insgesamt knapp über 60% war die Sicherung einer genetischen Diagnose möglich. Hierbei zeigte sich ein großes Spektrum unterschiedlicher kausaler Gene, davon 4 Varianten unklarer Signifikanz. Das phänotypische Spektrum zeigte neben klassischen Symptomen auch atypische Phänotypen sowie z.T. große interfamiliäre Variabilität. Schlussfolgerungen: Im CMT-Register Berlin zeigte sich ein diverses Spektrum an Genotypen und Phänotypen. Die genaue Beschreibung der Geno- und Phänotypen ist für die Entdeckung krankheitsmodifizierender Faktoren, Behandlung, Anbindung und Vorbereitung von klinischen Studien von großer Bedeutung.

Etablierung des deutsch-österreichischen LGMD-Patientenregisters

Abstract-Text (inkl. Referenzen) Gliedergürtel-Muskeldystrophien (LGMD) sind heterogene, genetisch bedingte Muskelerkrankungen mit zunehmender Schwäche und Atrophie der Schulter- und Beckengürtelmuskulatur. Zahlreiche Therapien befinden sich in der Entwicklung, bislang gibt es keine ursächliche Behandlung; die LGMD-Registerlandschaft ist fragmentiert, und es gibt noch keine ausreichenden subtypenspezifischen Daten zum natürlichen Verlauf der Erkrankung. Gemeinsam mit TREAT-NMD wurde ein gemeinsamer europäischer Registerdatensatz etabliert, der aktuell in den Mitgliedsländern gelauncht werden soll. Am Friedrich-Baur-Institut in München werden bereits verschiedene deutsch-österreichische und internationale Register für Patienten mit neuromuskulären Erkrankungen betrieben. Sie sind ein Meilenstein im Kampf gegen diese bislang nicht ursächlich heilbaren Erkrankungen. International hat man sich auf eine harmonisierte Liste an Daten für die einzelnen nationalen Registern geeinigt. Diese werden dann in anonymisierter Form in einer globalen Datenbank gespeichert, wo sie unter bestimmten Voraussetzungen zur Planung von klinischen Studien verwendet werden können. Über die nationalen Patientenregister kann die Patientenrekrutierung unterstützt werden. Wichtig ist die Einhaltung ethischer Grundsätze und der Schutz der wichtigen Patientendaten.

Natural history of boys with Duchenne muscular dystrophy (DMD) measured by timed function tests and mechanography

Abstract-Text (inkl. Referenzen) Introduction: Clinical trials targeting cohorts of boys with DMD over a several years are necessary to evaluate disease progression and drug efficacy. Boys with DMD often have gross motor delays early in life, and although they gain skills, they are on a lower trajectory than typical peers. In early school age losing of motor skills and later loss of ambulation are the natural course of the disease. In this study, we demonstrated natural history of boys with DMD (non-treated, treated with corticosteroids/ataluren) measured by timed function tests and mechanography. Methods: We define the natural history in boys with DMD aged from 3.1 to 18 years using the 6-minute walk test (6MWT), 10-meter run, time to stand up from a supine position and climb four stairs. Additionally, mechanography measured force in chair rising and jumping. Assessments were completed as standard of care during regularly scheduled clinic visits. Results: 77 boys with DMD were evaluated up to 21 follow up measurements available. Test battery included 6MWT (N=416), 10-m run (N=446), stand up from supine (N=13), climb four stairs (N=450), chair rising (N=228) and single two-legged jump (N=219). Longitudinal data are presented by age as a whole and in subcohorts of treated (corticosteroids, ataluren) and non-treated patients. Conclusions: Our study documents the natural history in a large cohort of boys with DMD. These data will be useful to compare ongoing and future therapeutic interventions for DMD.

Genotyp-Phänotyp-Spektrum idiopathischer Small Fiber Neuropathien

Abstract-Text (inkl. Referenzen) Einleitung: Small Fiber Neuropathien (SFN) sind ursächlich heterogen und oft idiopathisch. Gain-of-function-Mutationen in Genen, die für spannungsabhängige Natriumkanäle kodieren, sind mit Dysfunktion/Degeneration dünner Nervenfasern assoziiert. In dieser Registerstudie charakterisieren wir den Phänotyp idiopathischer SFN und suchen nach (neuen) genetischen Ursachen. Methode: Patient*innen mit idiopathischer/familiärer SFN erhalten Schmerzanamnese, PainDETECT, klinische Untersuchungen, Elektroneurographie, Quantitativ Sensorische Testung, Riechtests, Sudoscan, Labordiagnostik und whole exome sequencing. Hautbiopsien dienen der Diagnosestellung sowie zur Quantifizierung epidermaler Nervenfaserdichten. Ergebnisse: Rekrutierung bis 02/2023 (work in progress). Seit Mai 2020 wurden bisher 277 Erwachsene untersucht, davon 179 (♀:♂=121:58, 19-76 Jahre) eingeschlossen. 167/179 haben neuropathische Schmerzen als Leitsymptom. Sensible Plus- und Minussymptome sind distal betont, kommen aber in allen Körperregionen vor. 154/179 Betroffene haben autonome Symptome. Die Familienanamnese ist positiv in 46/175 Fällen. 18/159 (131 negativ, 11 übrig) bereits sequenzierten Patient*innen tragen eine Sequenzvariante in einem der SCN-Gene, deren Pathogenität derzeit evaluiert wird (8 SCN9A, 6 SCN10A und 5 SCN11A Varianten). Zusammenfassung: SFN sind heterogen und belastend. Im März 2023 werden wir aktuelle Ergebnisse aus der dann abgeschlossenen Registerstudie vorstellen.

Expanding the neuromuscular spectrum of DYNC1H1-associated disorders

Abstract-Text (inkl. Referenzen) Introduction: Neurons require dynamic intracellular trafficking networks in response to cellular stress. The dynein complex is essential for retrograde axonal transport of damaged components for autophagolysosomal processing. Pathogenic heterozygous variants in its core component, cytoplasmic dynein heavy chain (DYNC1H1), are associated with axonal motor neuropathy, including Charcot-Marie-Tooth disease type 2O (CMT2O). Axonal CMT2 neuropathies form a heterogeneous group with motor and/or sensory neuropathies, yet there are no reports of sensory neuropathies with DYNC1H1 variants to this date. Methods: We recruited patients through international collaborations and patient networks. We present clinical, molecular and imaging data on the age-related neuromuscular spectrum. Results: We followed up over 30 unreported cases with pathogenic variants in DYNC1H1 (ages 0-58 years). In addition to motor neuropathy, we identified several cases with novel age-related, progressive sensory neuropathy after the second decade of life, including tingling and numbness in patients with antecedent twitching, trembling, and tremors. We discuss DYNC1H1-related axonal motor sensory neuropathy in the context of CMT2 disorders and its life-long continuum in age-related progression of neuromuscular disorders. Conclusion: These findings extend the clinical, molecular and imaging spectrum of pathogenic DYNC1H1 variants, and facilitate genetic counselling and anticipatory guidance of patients.

Natural history of CPEO, CPEO plus and KSS – a trans-sectional and longitudinal analysis of the german mitoNET cohort

Abstract-Text (inkl. Referenzen) Introduction: CPEO, CPEO plus and Kearns-Sayre-Syndrome are a continuous spectrum of mitochondrial diseases with PEO, muscle weakness and variable affection of other body systems including heart, CNS and PNS. Most common cause is a single deletion of mtDNA, but other mtDNA mutations as well as nuclear mutations may be causal.In 2009 the German network for mitochondrial diseases (mitoNET) implemented a registry and a natural history study for patients with mitochondrial diseases (mitoREGISTRY). Methods: Out of &gt; 1500 patients in the mitoREGISTRY we have identified 275 patients with CPEO-, CPEO plus- or KSS-phenotype, some with a follow up of up to 10 years. We will analyze systematically all datasets and determine the genetic background, clinical manifestations over time and functional parameters evaluated by physical exam, NMDAS and SARA-Score. Results: The cohort consists of 275 patients, 150 females and 125 males, with a mean age of 47 years at baseline. Mean age at onset 24,7 years. We will show the spectrum of underlying genetic causes and clinical manifestations over time. Longitudinal data in terms of a mean yearly progression rate in NMDAS and SARA will be presented. Discussion: We will present natural history data of a large cohort of CPEO, CPEO plus and KSS patients with a follow up of up to 10 years. Natural history studies with NMDAS data and symptom development over time are important for prognostic counseling and can inform for design of future clinical trials.

Results from a nationwide health-related quality of life study in patients with spinal muscular atrophy

Abstract-Text (inkl. Referenzen) Objectives: To estimate self-assessed health related quality of life (HRQoL) of patients with spinal muscular atrophy (SMA). Methods: Online survey via national SMA registry in Germany. Estimates of self-reported HRQoL of the EQ-5D-5L rating scale (VAS score: visual analog scala) as well as utility score (preference-based estimate from members of the general population) were stratified by current best motor function (i.e., non-sitter, sitter, and walker). Results: A total of 82 adults (mean age 43 years, SD 15,06) and 7 children (mean age 15 years, SD 1,40) completed the questionnaire: 10% non-sitter (1 SMA1, 6 SMA2, 2 SMA3), 58% sitter (3 SMA1, 26 SMA2, 29 SMA3), 22% walker (21 SMA3). 75 % of participants were on disease specific medication. Mean VAS score for adult participants was: non-sitter 69,71 (SD 22,19), sitter 71,4 (SD 21,75) and walker 63,95 (SD 21,96). Higher incidence in pain/discomfort domain was found on walkers (32%) compared to sitters (19%). The mean total utility score was 0,51 (SD 0,27, range -0,55 – 1). Mean VAS score for children was: non-sitter 87,5 (SD 3,54), sitter 64,67 (SD 25,01), walker 90 (SD 0). The mean total utility score was 0.54 (SD: 0.31, range: 0.30–1.00). Conclusions: Estimates of self-reported HRQoL were higher in children than in adults with SMA, with exception of sitter group. As expected, mean utility scores were similar. Further deep analysis to better understand impact of specific disease therapies and disease burden are needed.
