Introduction: The identification of perpendicular vascular changes of the vocal folds (PVC) has emerged as an indicator of malignancy in the endoscopic assessment of glottic lesions. Classifying them further into wide and narrow-angle PVC aids in differentiating between papillomas and carcinomas. Accurate identification requires clinical experience. Artificial Intelligence (AI) algorithms can support endoscopic evaluations. Materials and Methods: Six physicians analyzed intraoperative images of vocal fold vasculature using Contact Endoscopy and Narrow Band Imaging (CE-NBI) post-training. The goal was to identify PVC and/or narrow-angle PVC as malignancy indicators. The same images were assessed using a published deep learning (DL)-based AI algorithm. Sensitivity and specificity of the approaches were compared. Results: A total of 5 CE-NBI images per patient were evaluated for 146 patients. Using PVC as a malignancy indicator, physicians achieved an average sensitivity of 78.6% and specificity of 69.1%. With narrow-angle PVC, the average sensitivity was 47.9%, and specificity was 86.1%. The AI-based evaluation achieved 100% sensitivity and 98.1% specificity. Conclusion: Identifying PVC in CE-NBI can indicate malignancy in evaluating glottic lesions of uncertain dignity, with lower sensitivity and higher specificity for narrow-angle PVC. An AI-based algorithm significantly enhances sensitivity and specificity in detecting malignant vocal fold lesions and can be a useful tool for diagnostic and training purposes.Einleitung: Die Identifikation von perpendikulären Gefäßveränderungen der Stimmlippen (PGV) gilt als Malignitäts-Indikator bei der endoskopischen Beurteilung glottischer Läsionen. Die Einteilung in weit- und engwinklige PGV dient der differenzierten Diagnose von Papillomen und Karzinomen. Eine genaue Zuordnung erfordert klinische Erfahrung. Automatisierte Künstliche-Intelligenz (KI)-Algorithmen können die endoskopische Auswertung unterstützen. Material und Methoden: Sechs Ärzte analysierten intraoperative Aufnahmen von Stimmlippengefäßen mittels Kontaktendoskopie und Narrow Band Imaging (KE-NBI) nach Schulung. Ziel war die Identifikation von PGV und/oder engwinkligen PGV als Malignitäts-Indikatoren. Die gleichen Aufnahmen wurden durch einen bereits veröffentlichten, auf Deep Learning basierenden KI-Algorithmus ausgewertet. Sensitivität und Spezifität der Ansätze wurden verglichen. Ergebnisse: Insgesamt wurden 5 KE-NBI-Aufnahmen pro Patient bei 146 Patienten ausgewertet. PGV als Malignitäts-Indikator zeigte durchschnittliche Sensitivität von 78,6% und Spezifität von 69,1%. Bei engwinkligen PGV lag die Sensitivität bei 47,9%, die Spezifität bei 86,1%. Der KI-Algorithmus erreichte 100% Sensitivität und 98,1% Spezifität. Schlussfolgerung: PGV in der KE-NBI können als Malignitäts-Indikator bei glottischen Läsionen dienen, wobei die Sensitivität bei engwinkligen PGV niedriger und die Spezifität höher ausfällt. Digital speech recognition in patients after laryngectomy – A prospective controlled study to assess the success of therapy

Introduction: Laryngectomy with voice rehabilitation is an established standard therapy for advanced laryngeal and hypopharyngeal carcinomas. Although a uniform assessment of the success of treatment with regard to voice rehabilitation is desirable, it has so far been limited to subjective assessments of speech intelligibility. The present study aims to establish an objective and comparable examination of speech intelligibility using digital speech recognition software. Methods: 20 patients after laryngectomy with voice prosthesis insertion and 20 age-matched control patients without laryngeal pathology were included. For the voice recordings, each patient was given a list of 20 single words, 20 multi-word sentences and a full text of 98 words to read aloud. The texts were generated by a speech recognition program and the recognized words were scored using a point system. Non-professional examiners then received the recorded speech recordings for a subjective evaluation, the results of which were compared with the objective measurements. In addition, questionnaires were completed to assess voice-related quality of life. Results: The objective results in speech recognition showed a good correlation with the subjective assessment regarding the number of recognized words. It also very well depicted the expectedly large variance of speech performance between the Patients with laryngectomy. Furthermore, the ratings of voice-related quality of life also correspond well with objective and subjective speech comprehension. Discussion: Digital speech recognition appears to be well suited for the objective assessment of speech intelligibility in laryngectomized patients for evaluating the functional success of therapy.Einleitung: Die Laryngektomie mit Stimmrehabilitation ist eine etablierte Standardtherapie fortgeschrittener Larynx- und Hypopharynxkarzinome. Auch wenn eine einheitliche Beurteilung des Therapieerfolgs hinsichtlich der Stimmrehabilitation wünschenswert wäre, ist diese bisher auf eine subjektive Beurteilung der Sprachverständlichkeit beschränkt. In der vorliegenden Studie soll eine objektive und vergleichbare Untersuchung der Sprachverständlichkeit mit einer digitalen Spracherkennungssoftware etabliert werden. Methoden: 20 Patienten nach Laryngektomie mit Stimmprotheseneinlage und 20 altersgematchte Kontrollpatienten ohne Larynxpathologie wurde untersucht. Für die Sprachaufnahmen erhielt jeder Patient 20 Einzelwörter, 20 Mehrwortsätzen und einen Volltext mit 98 Wörtern zum Vorlesen. Die Texte wurden durch ein Spracherkennungsprogramm generiert und anhand eines Punktesystems wurden die erkannten Wörter bewertet. Anschließend erhielten nicht-professionelle Untersucher die aufgezeichneten Sprachaufnahmen für eine subjektive Bewertung, deren Ergebnisse mit den objektiven Messungen verglichen wurden. Zudem wurden Fragebögen zur Beurteilung der stimmbezogenen Lebensqualität ausgefüllt. Ergebnisse: Die Ergebnisse des objektiven Testsettings korrelierten sehr gut mit den subjektiv ermittelten Verständnisquoten und konnte auch die erwartbar große Spannbreite an Stimmergebnissen bei den Laryngektomierten gut abbilden. Auch die Bewertungen der stimmbezogenen Lebensqualität standen mit objektivem und subjektivem Sprachverständnis in Einklang. Diskussion: Zur objektiven Beurteilung der Sprachverständlichkeit bei laryngektomierten Patienten scheint die digitale Spracherkennung ein gut geeignetes und nützliches Tool zur Bewertung des funktionellen Therapieerfolgs zu sein.Die Autoren geben an, dass kein Interessenkonflikt besteht.

Treatment per DeLOS-II protocol improves outcome of patients with locally advanced Laryngeal and Hypopharyngeal squamous cell carcinoma compared to conventional regimens

Verbessertes Langzeitüberleben durch Therapie nach DeLOS-II-Protokoll bei Patienten mit lokal fortgeschrittenen Karzinomen des Larynx/Hypopharynx verglichen mit konventionellen multimodalen Konzepten

Introduction: Advanced laryngeal and hypopharyngeal squamous cell carcinoma (LA-LHSCC) and its treatment can lead to impaired function or ablative surgery (total laryngectomy, TL). 10 years after closing the larynx organ-preservation trial DeLOS-II, their long term follow-up data was compared to propensity score (PS)-matched controls undergoing conventional treatment at the University Hospital Leipzig. Material/Methods: DeLOS-II used TPF-induction chemotherapy (IC) followed by radiotherapy (RT) ± cetuximab for responders according to endoscopically estimated tumor surface shrinkage ≥ 30% after cycle 1. Non-responders received TL plus postoperative radio-(chemo)-therapy (TL+POR(C)T). Follow-up data of 382 LA-LHSCC including n=52 DeLOS-II patients treated at the University Hospital Leipzig from electronic health records and the Saxonian cancer registry were analyzed using SPSS. Overall (OS) and tumor-specific survival (TSS) was assessed after PS-matching of DeLOS-II- and 52 patients each treated by either chemoradiation (CRT), TL+PORT or TL+PORCT considering main risk factors. Results: OS and TSS were superior in DeLOS-II patients (p= .045, p= .022). Their mean OS was 87.7 (95% CI 70.8-104.6) months, median OS 86.2 (95% CI 39.0-133.4) months; mean TSS 143.0 (95% CI 122.1-163.9) months, median not reached. Comparing the 4 treatment protocols directly, OS and TSS in DeLOS-II were improved but not in general, i.e. compared to TL+PORCT (p= .349; p= .076), TL+PORT (p= .049, p= .131) or CRT (p= .024; p= .007). Conclusion: DeLOS-II patients had superior OS and TSS compared to patients treated with TL+PORCT, TL+PORT or CRT. A disease-specific gain in survival from DeLOS-II was observed but decreased over time. Effect of prophylactic HPV-vaccination (Gardasil®) on recurrent respiratory papillomatosis

Introduction Since 2007, the Standing Committee on Vaccination (STIKO) has recommended prophylactic vaccination to prevent human papillomavirus (HPV) infection. Possible therapeutic benefits are discussed. In this study, the effect of vaccination on the course of recurrent respiratory papillomatosis (RRP) is examined. Methods Files of 63 RRP patients (21 females, 43 males) treated at the Kiel ENT Clinic from 2008 to 2021 were retrospectively reviewed. The burden of disease was defined by ablations per year and the summed anatomical Derkay score related to the treatment period in years (DerkaySumme). Results 18/63 patients (28.6%; 10 females, 8 males) were vaccinated against HPV. All vaccination time points were available in 10 patients (55.6%). Considering vaccination data with the maximum observation period for each patient, ablations per year significantly decreased from a median of 1.72 to 0.33 (p &lt; 0.01; n = 11). The DerkaySumme also significantly decreased from a median of 5.17 to 3.04 (n = 9; p &lt; 0.05). Among the unvaccinated (n = 14), ablations per year in the second half of the disease (M = 0.5; Mdn = 0.31) were comparable to those in the first half (M = 0.5; Mdn = 0.29; p &gt; 0.05). Discussion HPV vaccination in RRP is significantly associated with a reduction in disease burden, demonstrated by decreased ablations and reduced tumor spread, as measured by the anatomical Derkay score. This should be further examined in larger groups to determine whether HPV vaccination for RRP should be standardized as a therapeutic option, even in the early stages. In addition, the therapeutic effect of vaccination should be investigated for other HPV-associated lesions.Einleitung Seit 2007 empfiehlt die ständige Impfkommission (STIKO) eine prophylaktische Impfung zur Verhinderung einer Infektion mit humanen Papillomviren (HPV). Ein möglicher therapeutischer Nutzen wird diskutiert. In dieser Untersuchung wird der Effekt der Impfung auf den Verlauf der rezidivierenden respiratorische Papillomatose (RRP) betrachtet. Methoden Akten von 63 RRP-Patienten (21 Frauen, 43 Männer), die von 2008 bis 2021 an der Kieler HNO-Klinik behandelt worden sind, wurden retrospektiv ausgewertet. Die Krankheitslast wurde definiert durch Abtragungen/Jahr sowie dem aufsummierten anatomischen Derkay-Score bezogen auf den Behandlungszeitraum in Jahren (DerkaySumme). Ergebnisse 18/63 Patienten (28,6 %; 10 Frauen, 8 Männer) waren gegen HPV geimpft. Bei 10 Patienten (55,6 %) lagen alle Impfzeitpunkte vor. Betrachtet man für jeden Patienten die Daten der Impfung mit maximalem Beobachtungszeitraum, sinken Abtragungen/Jahr im Median von 1,72 auf 0,33 statistisch signifikant (p &lt; 0,01; n = 11). Der DerkaySumme sinkt im Median von 5,17 auf 3,04 statistisch signifikant (n = 9; p &lt; 0,05). Bei Ungeimpften (n = 14) sind die Abtragungen/Jahr in der zweiten Hälfte der Erkrankung (M = 0,5; Mdn = 0,31) vergleichbar zur ersten Hälfte (M = 0,5; Mdn = 0,29; p &gt; 0,05). Diskussion Eine HPV-Impfung bei RRP ist signifikant assoziiert mit der Reduktion der Krankheitslast im Sinne verringerter Abtragungen sowie geringerer Tumorausdehnung, objektiviert durch den anatomischen Derkay-Score. Dies sollte in größeren Gruppen überprüft werden, um zu erkennen, ob eine HPV-Impfung bei RRP bereits im frühen Stadium als therapeutische Option standardisiert Anwendung finden sollte. Visualisation of pharyngo-esophageal spasm while performing botulinum therapy in patients with tracheo-esophageal prostheses

Aims: By using different methods to visualise pharyngospasm, to achieve the clinical effect of botulinum therapy and to reduce the dose of the drug. Materials and methods: We observed 6 patients with voice prosthesis dysfunction who underwent total laryngectomy with tracheo-oesophageal puncture (TEP). Two of the patients underwent intraoperative TEP, while the other four underwent delayed TEP (3 to 6 months). All six patients received radiotherapy after surgery. One of the patients was diagnosed with a recurrence of the disease, the other two had severe pharyngeal stricture. The remaining three patients were diagnosed with pharyngospasm using real-time MRI with oral contrast. The treatment of choice was chemical denervation with botulinotoxin A. Results and discussion: Botulinum toxin A injection was performed in all three patients under ultrasound guidance to visualise the localisation of the spasm. The dose of the drug used was 40 OG, which is 10 points less than the usual minimum dose. The clinical effect was assessed by the speech therapist 72 hours later. A real-time MRI control was performed, which showed adequate dilatation of the pharyngo-esophageal segment. After 6 months of follow-up, all patients had significant improvement of tracheo-oesophageal speech. Conclusions: The use of real-time MRI in the diagnosis of spasm of the pharyngo-esophageal segment, together with ultrasound-guided botulotoxin injection precisely at the site of the spasm, gives the possibility to achieve a stable clinical effect and to reduce the dose of the toxin.The authors declare that there is no conflict of interest

A minimally invasive laryngeal surgical robot

The da Vinci XI and SP surgical robots cannot adapt to the existing minimally invasive laryngeal surgery, and have disadvantages such as poor exposure and thick robotic arms. The surgical robot made in China dedicated to minimally invasive throat can solve the current problem very well.
