Nasopharyngeal Warthin's tumor: A case report and literature review

Abstract

Introduction: Warthin´s tumor, also known as papillary cystic adenolymphoma, is the second most common benign salivary tumor after pleomophic adenoma. It occurs predominantly in the parotid gland. Extraparotideal Warthin´s tumors are rare. Material/Methods: We present a case of a 66 year-old smoking women with an incidentally detected Warthin´s tumor of the right tubal protuberance. Biopsy was performed via a transnasal endoscopic access. The case report is complemented by a literature review of previously published cases of Wartin´s tumors of the nasopharynx. Results: Literature revealed 20 other published cases of nasopharyngeal Warthin´s tumors. As far as it could be determined, the tumors were diagnosed between the fifth and eighth decade of life, men were affected about twice as often as women and at least nine patients indicated smoking. The most common symptoms were impaired hearing, nasal discharge or nasal congestion, although a proportion of patients did not report any symptoms. In seven cases a synchronous Warthin´s tumor of the parotid gland was found. Discussion: Nasopharyngeal Warthin´s tumors are exceptionally rare, but should be considered as a possible differential diagnosis, especially in a patient population aged 50 to 80 with a nasopharyngeal tumor. Histological confirmation is always indicated. Surgical excision can be offered when symptoms occur.
