IntroductionThe current configuration of the national healthcare system in Italy tends to create peripheral hospitals for the primary treatment of surgical pathologies, alongside tertiary centers for specialized care. This leads to the belief that the general surgeon is destined for peripheral hospitals, while the hyper-specialist surgeon operates in dedicated centers. The aim of this study is to identify the possible influence of this belief on the training of surgeons, by the use of a questionnaire administered to a representative sample of specialist surgeons to investigate whether the general surgeon figure is still relevant.Material and MethodsThis study analyzes, for the first time in Italy, the perception of the figure and the transversal training of the general surgeon, which may seem anachronistic compared to hyper-specialization. The most significant data came from the analysis of an ordered logit model, which examined the question "Which factors can help us perceive the transversal surgical training as always current?" The answer is that the general surgeon's versatility, both educational and professional, is indeed fundamental and current.Results and ConclusionsOur findings suggest the need to modernize the training process within surgical specialties to ensure a strong and reliable foundation upon which high-quality professional training can be built.A standardization of training common to all schools would be desirable, allowing everyone to reach a baseline level of surgical exposure. Implementing a training program based on the acquisition of standardized theoretical and practical skills, more detailed than the current one, stratified by level and year of specialization, with genuine attention to the surgical curriculum, are all aspects that could renew and enhance the training system of the General Surgery Specialization School in Italy.No

The challenges and the dilemma in surgical resident training of millennials

Surgical residency programs are demanding and rigorous. They involve years of intense training, long working hours, and high levels of stress. Millennials are confident and place a high value on work-life balance which clashes with demanding surgical training steeped in tradition and hierarchy. The core surgical training method is the one-on-one master-apprentice relationship, which has been passed down through generations. This relationship requires residents' subordination in exchange for knowledge and a chance to be entrusted with the opportunity to perform surgical procedures. Millennials expertly use their smartphones to multitask and interact with their peers while being oblivious that older surgeons perceive their constant use of phones as rude. They do not recognize hierarchy to the same degree as senior generations. Millennials are often not willing to accept negative or even constructive criticism because they were raised with positive reinforcement. They are used to immediate access to information and are less likely to read textbooks or attend lectures. Many millennials opt not to pursue a surgical career as we are noticing declining interest and vacant slots in surgical disciplines. With many mentors not willing to change the time-honoured training of residents we are faced with the dilemma: should traditional training be changed to accommodate millennials, or should surgical specialties be content with fewer applicants/residents? What if in the end we are left with none? Conclusion Compromise. As no innovative technology can substitute hands-on training and manual skills, efforts to improve training are crucial for retaining talented residents and attracting new applicants. Providing immediate feedback, defining expected conduct, and rules of acceptable behaviour can make a difference. Helping residents realize that work may at times take precedence over their personal lives as well as helping mentors understand there is life outside of hospitals.NO

Opiate addiction is a prevalent challenge in trauma surgery, where postoperative pain management often necessitates opioid use. This presentation explores an integrated approach to treating opioid addiction following trauma surgery, emphasizing the importance of early intervention and collaboration between surgical and addiction psychiatry teams. We will examine protocols for assessing risk, initiating addiction treatment, and managing pain without compromising recovery. Through case studies and evidence-based practices, this discussion aims to enhance awareness and promote strategies that reduce dependency risk, ensuring a holistic, patient-centered recovery pathway for trauma patients in post-surgical care.No.

Assessment of knowledge retention on basic life support among second-year medical students one year after training

Introduction: Basic Life Support (BLS) comprises procedures for out-of-hospital cases of cardiopulmonary arrest (CPA) or airway obstruction. Key BLS components include Cardiopulmonary Resuscitation (CPR), to maintain blood flow to vital organs until circulation is restored, and airway clearance maneuvers. Performed by trained individuals at the scene, BLS can triple survival chances. Given its importance, a university extension group provided BLS training to first-year medical students in 2023, assessing knowledge retention a year later to identify gaps and suggest improvements in academic training.Material & Methods: In August 2024, second-year students who completed the training took a questionnaire with 10 theoretical questions (identical to pre and post-training tests from 2023) and 5 demographic questions. Students rated their confidence on a scale from 1 to 10. Results were compared to 2023 test data for longitudinal analysis.Results: A total of 64 responses were obtained, from participants aged 18 to 27, 95.31% of whom were in their first degree. Of these, 21.8% had experienced BLS situations. The average score was 66.25%, higher than the pre-training test (50.31%) but lower than the post-training test (95.35%). The largest drop in accuracy occurred in questions on compression rate, CPR cycle rotation and AED (Automatic External Defibrillator) definition. A limitation was the lack of practical skill assessment.Conclusions: The research showed strong student engagement, emphasizing the importance of BLS knowledge. Educational intervention with app-based motorcycle delivery drivers in a Yellow May campaign – A collaborative effort between the municipal development and traffic authority of Campinas, Brazil, a university extension project, and Brazil's leading food delivery app

Introduction: In Brazil, approximately 1 million motorcyclists work as delivery drivers. The speed and pressure to meet deadlines raise collision risks, especially in urban areas. The push to complete more deliveries leads to risky behaviors like speeding, phone use and unsafe maneuvers, making delivery motorcyclists more prone to severe accidents. A university-led "Trauma Prevention" extension project partnered with the Campinas Municipal Development Company (EMDEC) to raise awareness among app-based food delivery motorcyclists (from Ifood, a major Brazilian platform) about traffic risks, documenting an action during the Yellow May campaign in 2024.Material & Methods: The activity at EMDEC involved motorcyclists from a popular Brazilian delivery platform, who provided informed consent and filled out a profile questionnaire. They participated in a 10-minute simulation of traffic situations using an Ifood simulator, followed by guidance on posture, braking and accident response from traffic agents and extension students.Results: A total of 33 motorcyclists participated, mostly men aged 25-50. Of these, 63.6% work exclusively with delivery apps, and 84.8% work 5 or more times weekly, mainly at night. Regarding accidents, 63.6% had experienced one while working. Although 75.76% knew how to act in accidents, only 33.33% would call Emergency Services first. Concerning phone use, 36.4% reported using it to locate clients. In accident situations, only 6% have medical and financial support. During training, participants engaged in a traffic simulation and received feedback on driving techniques and accident response.Conclusions: The participants' profile highlights this group's vulnerability. Comparison between ATLS and ETC: Impact of trauma academic leagues on interdisciplinary training for trauma care

The Advanced Trauma Life Support (ATLS) and European Trauma Course (ETC) programs are essential in enhancing trauma victim care, with Trauma Academic Leagues playing a key role by connecting students to these methodological training programs. OBJECTIVE: This study aims to compare the approaches of the ATLS and ETC programs, analyzing their impact on the interdisciplinary training promoted by Trauma Academic Leagues. METHODOLOGY: A literature review was conducted covering the period from 2018 to 2023, using the PubMed and Scielo databases, and selecting three relevant articles on these methodologies and the role of academic leagues in trauma training. DISCUSSION: The ATLS program follows a hierarchical structure, with centralized medical leadership and specific roles for other professionals, while the ETC emphasizes simultaneous collaboration, promoting interaction among various health fields and non-technical skills, such as communication and coordination. By implementing these methodologies, Trauma Academic Leagues enable students to develop both technical and collaborative competencies. CONCLUSION: Trauma Academic Leagues facilitate the integration of these approaches. Yellow wave: Impact of a structured educational initiative on trauma prevention through social engagement of medical students at ESTES congress 2024 – Portugal

Introduction: Yellow Wave (YW) is a collaborative initiative aimed at promoting the YELLOW MAY prevention program and producing scientific papers for the 23rd European Trauma Congress. The project brings together students, surgical residents, and professors to address trauma prevention, while also offering students from disadvantaged backgrounds the opportunity to showcase their research at an international congress.Material & Methods: This longitudinal prospective study had two phases. The intervention pre-congress protocol comprised structured scientific mentorship, research methodology training, and presentation skills development. After the congress, an electronic questionnaire was administered, assessing five primary domains. Primary outcomes measured changes in academic skills through paired pre-post assessments, while secondary outcomes included social engagement metrics and knowledge accessibility indicators.Results: Onda Amarela enrolled 58 participants and 21 professors, 41 students responded. Significant correlations emerged in research engagement (p=0.028) and prevention activities participation (p=0.038)(95%), like P.A.R.T.Y. program, mass casualty simulation, Yellow May campaign and ATLS®. In addition, 65% of them aimed to pursue a general surgery career, and 62% reported positive impact on their academic career aspirations (p=0.006).Conclusion: The YW initiative greatly improved students" research involvement, skill development, career aspirations, sparking an interest in general surgery careers, and social engagement. Heterotopic pancreatic tissue presenting as recurrent biliary colic

Purpose This case report aims to describe the presentation, diagnostic approach, and management of heterotopic pancreatic tissue in the gallbladder wall, an uncommon condition that can mimic biliary colic in the absence of gallstones. The goal is to highlight this entity as a differential diagnosis in patients with unexplained biliary symptoms and a thickened gallbladder wall.MethodsA woman in her early 40s presented with a two-year history of recurrent vomiting and upper abdominal pain, unrelated to food intake. Clinical evaluation suggested biliary colic, leading to additional investigations. Oesophagogastroduodenoscopy (OGD) and imaging studies, including ultrasound and magnetic resonance cholangiopancreatography (MRCP), showed diffuse thickening of the gallbladder wall without stones, suggestive of chronic cholecystitis or a potential neoplastic process. The patient underwent laparoscopic cholecystectomy, and histopathology confirmed heterotopic pancreatic tissue in the gallbladder neck.ResultsHistopathology revealed chronic inflammation, muscular hypertrophy, and heterotopic pancreatic tissue within the gallbladder wall. Following surgery, the patient experienced complete symptom relief, with no recurrence of abdominal pain or vomiting one year postoperatively.ConclusionThis case illustrates the importance of including heterotopic pancreatic tissue as a differential diagnosis for recurrent biliary colic, especially when imaging shows gallbladder wall thickening without stones. Although extremely rare, pancreatic tissue heterotopy in the gallbladder can mimic gallbladder malignancy, emphasizing the need for histological examination after cholecystectomy for an accurate diagnosis. Clinicians should remain aware of this condition, as it may prevent unnecessary concern over suspected malignancy and guide appropriate treatment.No.
