Abstract

Introduction: Currently, there is no sensitive and specific marker for the early detection and follow-up of laryngeal carcinomas. Hence, the objective of this study was to solve this shortcoming and 1) identify potential non-invasive biomarkers as well as 2) follow them prospectively over time.

Methods: Firstly, serum from an identification cohort (n=49 controls, n=75 laryngeal squamous cell carcinomas (LSCC) UICC I-IV) was used to identify potential biomarkers in a two-step approach (multiplexed assay, ELISA). Selected serum biomarkers were then prospectively followed in a validation cohort of n=59 LSCC patients (preoperatively, postoperatively, follow-up). This is a preliminary analysis, the study is ongoing.

Results: Out of all 2000 proteins, the 10 most promising markers were identified. The biosignature consisting of Periostin, IGFBP7, Nov and uPAR was most promising and significantly identified early stages of LSCC (UICC I, II, e.g. T1/2 p<0.045, T3/4 p<0.001). Additionally, the biosignature changed significantly during the follow-up (p<0.05 for all).

Conclusion: The investigated non-invasive markers seem to be promising for the early detection of LSCC. This preliminary analysis also hints to the potential of the biosignature in the follow-up period of the disease. However, for determining the clinical meaningfulness in terms of prediction of severity and recurrences, a larger cohort with a longer follow-up is necessary.
