Introduction: Extramedullary manifestation in the larynx is a very rare localisation of multiple myeloma. The malignant disease is categorised as a B-cell non-Hodgkin's lymphoma. A distinction is made between extramedullary plasmacytoma without bone marrow involvement and the extremely rare occurrence of the extramedullary form in the head and neck area as part of multiple myeloma.

Patient: We describe a 57-year-old patient with an acute subglottic, oedematous swelling and progressive dyspnoea, which led to an airway securing by tracheotomy. A CT scan showed a cartilage destruction of the cricoid cartilage with an almost completely obstructed airway. The patient had a history of smouldering myeloma of the IgG kappa type for 4 years. (CRAB criteria negative, plasma cell infiltration of 15-20% in the bone marrow)

Results: With initial radiological suspicion of a malignant finding in the cricoid cartilage, a sample was taken under general anaesthesia. Histological examination revealed a marginal infiltration of a myeloma with kappa light chain restriction. In addition, the subsequent PET-CT examination revealed focal osteolysis in the clavicle and femoral head. The patient was therefore diagnosed with multiple myeloma with extramedullary manifestation and oncological treatment was started. The oedematous swelling was completely regressed in the endoscopy 2 months after initiation of therapy, and the tracheostomy was closed promptly.

Conclusion: Due to the subglottic swelling, the patient's diagnosis of multiple myeloma with extramedullary manifestation requiring treatment was confirmed and the decision to start immunotherapy/chemotherapy followed by autologous stem cell transplantation was made in the interdisciplinary tumour board in accordance with the guidelines.

Einleitung: Die extramedulläre Manifestation im Larynx ist eine sehr seltene Lokalisation des multiplen Myeloms (MM). Die maligne Erkrankung zählt zu den B-Zell Non-Hodgkin-Lymphomen. Unterschieden wird das extramedulläre Plasmozytom ohne Knochenmarksbefall sowie das im Kopf-Hals-Bereich extrem seltene Auftreten der extramedullären Form im Rahmen eines MM. Patient: Wir beschreiben einen 57-jährigen Patienten mit einer akuten subglottischen, ödematösen Schwellung und progredienter Dyspnoe, welche im Verlauf zu einer Tracheotomie führte. Im CT zeigte sich eine Knorpeldestruktion des Ringknorpels mit nahezu vollständig verlegtem Atemweg. Anamnestisch bestand seit 4 Jahren ein Smouldering Myeloma Typ IgG kappa. (CRAB-Kriterien negativ, Plasmazellinfiltrationsanteil 15-20% im Knochenmark) Ergebnis: Bei initial radiologischem Verdacht eines malignen Befundes des Ringknorpels erfolgte die PE in Intubationsnarkose. Es konnte histologisch eine Infiltration eines Myeloms mit Kappa-Leichtketten-Restriktion festgestellt werden. Zudem zeigten sich in der anschließenden PET-CT fokale Osteolysen in Clavicula und Femurkopf. Somit konnte bei dem Patienten die Diagnose eines multiplen Myeloms mit extramedullärer Manifestation gestellt und eine onkologische Therapie begonnen werden. Die ödematöse Schwellung war 2 Monate nach Therapieeinleitung in der Endoskopie komplett regredient, das Tracheostoma konnte zeitnah verschlossen werden. Schlussfolgerung: Hervorgerufen durch die subglottische Schwellung konnte bei dem Patienten die Diagnose eines therapiebedürftigen multiplen Myeloms mit extramedullärer Manifestation gesichert und interdisziplinär im Tumorboard leitliniengerecht die Entscheidung zur Immun-/Chemotherapie mit anschließender autologer Stammzelltransplantation getroffen werden.
